Internal Medicine or Family Medicine Physician – Post Acute CareSeeking a Board-Certified Family Medicine, Internal Medicine physician to join a practice providing primary care to adults in skilled nursing facilities and inpatient rehab.Position Details : Full-time positionFlexible schedule. Set your own schedule.Estimated encounters per day 15-20Day call only on days covering every weekendJob Duties : Primary care rounding in SNF.Manage chronic disease, diagnosis illness and medical conditions, and implement plan of care.Collaborate with clinicians, facility leaders and others regarding care and services.Build relationships with your patients, patients’ family members, and facility staff to facilitate effective treatment delivery.Provide oversight, guidance, and mentorship to our cliniciansConduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care.Represent company on appropriate external levels engaging and establishing / maintaining relationships within the post-acute care community.Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues.Compensation and Benefits : Compensation range $181,000 - $261,000Comprehensive benefits package including : Medical InsuranceDental insuranceVisionLife insurance401k with matchMalpractice including tail#J-18808-Ljbffr
