Physician / MD qualifications required) Internal Medicine - Seeking a Board Certified Internal Medicine physician to join a practice working with patients in all levels of post-acute care including LTAC, sub-acute rehab, and inpatient rehab.

Responsibilities :
Work alongside pharmacists on medication management
Complete Medicare required visits
Complete acute, follow up, and discharge visits as necessary
Implement an individualized plan of care for patients' goals
In collaboration with the IDT determine transitional care
Make recommendations on DME, home health services, or outpatient services after short stay.
Will not accept visa candidates.
