The Homeless and Housing Programs Manager oversees Neighborcare Health’s health care clinics and services for individuals currently or formerly homeless, residing in low-income housing, and those with complex health care needs, which currently includes the following clinics:

• Ballard Homeless Clinic at Nyer Urness

• Homeless Youth Clinic at New Horizons

• Youth Clinic at Pike Place

They provide strategic guidance to clinic and program administrators and their staff in alignment with our service commitments – a warm welcome, respect, working together, trusting relationships and caring – grounded in our core values of social justice and community. In partnership with program staff and organizational leaders, the Homeless and Housing Programs Manager also oversees personnel management and all program operations.

The Homeless and Housing Programs Manager is the content expert in the organization on the philosophy and strategic direction of targeted services for people experiencing homelessness and clinics located within housing and community-based organizations. They lead and manage the local and national Health Care for the Homeless Network partnerships and grant compliance, working closely with the Pike Market Clinic Administrator, Housing & Street Outreach Program Manager and Lake City/Magnuson Clinic Administrator on alignment of the outreach and clinic services delivered at Neighborcare Health.

Program Oversight

• Develop and guide the strategic vision for Homeless and Housing programs in collaboration with program staff and in alignment with Neighborcare Health’s organizational strategic goals

• Research and develop innovative models of care to meet the needs of homeless and housing programs patients and enhance the triple aim of better care, increased access, and lower cost

• Build and maintain partnerships with community organizations with whom Homeless & Housing programs coordinate to provide services, including strategic planning, collaborative funding opportunities, MOU/BAA completion and conflict resolution

• Establish and maintain effective working relationships with internal Neighborcare departments and external referral sources, community partners, and key stakeholders, including organizations where homeless and housing programs staff provide regular on-site services

• Work with other Neighborcare Health leaders to achieve mission and guiding principles; seek out and value input of the team

Staff Management

• Build teams and motivate staff; provide direct supervision and professional development to H&H Clinic leads; facilitate collaborative learning and conduct annual Learning Conversations

• Work with lead team and staff to develop annual goals and objectives to improve operations and report regularly on progress

• Provide mentorship to H&H clinical supervisors – SMD, BH, RN - regarding population, philosophy of care, and partnership best practices; support with resolving issues related to this specialized body of work

• Facilitate Homeless & Housing Leads meetings; serve as a convener, integrator and decision maker in areas where the H&H Clinics and Pike Clinic/Housing & Street Outreach teams overlap

• Lead the H&H Patient Navigation team in partnership with direct site supervisors and social work supervisor, as needed

Budget Management

• Build and monitor overall program budgets and productivity performance in partnership with Homeless & Housing Clinic leads

• Create, implement, and monitor evaluation and quality improvement efforts in collaboration with site, funder and organizational strategic goals; communicate QI results, financial results, customer satisfaction, staff satisfaction results to clinic team

• Assure required grant/funder reporting and invoicing is completed and submitted in partnership with program staff and Development Department.

• Coordinate opportunities for funding with Homeless and Housing programs staff, leadership, and Development Department

KEY SKILLS, KNOWLEDGE, & ABILITIES

Required for this job (List skills, knowledge, and abilities preferred for this job):

• Previous experience providing direct service to individuals and/or families experiencing homelessness

• Previous experience working with grant funded programs

• Knowledge of the homeless services continuum in the greater Seattle area and familiarity with key partners

• Knowledge of and ability to demonstrate excellent organization, communication and follow through skills

• Computer experience: windows, word processing, spreadsheets

• Ability and means to commute during the day to various locations

• Ability to manage, guide and direct team to meet organization, clinic and department goals and objectives

• Ability to read, write and communicate in English and use of math skills

• Ability to meet and comply with HIPAA/Confidentiality policies and procedures and ability to handle highly confidential and sensitive patient information and staff member information

• Ability to develop, maintain rapport and work effective with a wide range of individuals

• Ability to handle multiple tasks at the same time including under stressful conditions

• Ability to effectively perform job duties under high levels of activity, variety of conditions and restraints

• Ability to work resourcefully and problem solve creatively in the absence of detailed instructions

• Ability to communicate effectively and respectfully with people of varied racial, educational and socio-economic backgrounds as well as individuals

• Ability to demonstrate excellent verbal and written communications skills

• Ability to remain calm and professional when faced with difficult situations or emergencies

• Ability to work comfortable and productively with diverse staff

• Ability to demonstrate predictable, reliable and timely attendance

 

Preferred for this job (List skills, knowledge, and abilities preferred for this job):

• Healthcare management/supervisory experience preferred, specifically the provision of care in nontraditional settings

• Working knowledge of trauma informed care, harm reduction, and motivational interviewing

• Previous management of federally funded projects or programs

 

EDUCATION

Required: High School Diploma/GED and five years relevant work experience in a professional setting

Preferred: Bachelor’s or Master’s degree in business or healthcare administration, health care, or health sciences

Years of Relevant Experience Required:

 

Two (2) years of direct supervision of people or teams experience

Years of Relevant Experience Preferred:

Two (2) years of health care administration or practice management

Two (2) years of leadership team supervision and development

Compensation 

  • The wage range for this position is minimum $79,892.80 up to $110,344.00 maximum.  
  • Compensation will be determined based on years of relevant experience.
