Position Summary: The Case Manager of Family Promise of Greater Lorain is responsible for coordinating services for individuals and families experiencing homelessness or at risk of homelessness in Lorain County through Homelessness Prevention, Diversion and Housing Retention. Responsibilities include taking and returning phones calls and/or referrals in relation to the above mentioned programming, creating case plans when needed, providing advocacy, funding and resources on a case-by-case basis.

Essential Job Functions:

  • Serves as the access point to Family Promise related services for Lorain County.
  • Answers and returns phone calls within a timely manner to assist people experiencing homelessness or at risk of homelessness with customized resources
  • Creates intensive, individualized case management plans by coordinating client goals with available supports and resources
  • Assesses and documents individuals for program eligibility.
  • Provides information and referrals to community resources, including mental health, AODA, medical, educational, financial services, based on assessment.
  • Advocates with family members and/or landlords to assist in problem resolution
  • Completes requests for funding to assist clients in maintaining current placements
  • Ensures case records are available for generating reports, audits, grant writing, supervision.
  • Uses Case Management tool to track statistical and geographical information, which is not entered into HMIS/Clarity for monthly reporting.
  • Maintains current roster of participants and follows up on a monthly basis, at minimum, to ensure housing stability
  • Encourages and supports participants in locating employment and gaining or maintaining permanent housing
  • Collaborates with community partners to provide quality programming for participants

· Attends meetings on behalf of the agency, including CoC, PSH, RRH to represent clients

· Follows up to ensure that clients are connected to and utilize referral services.

  • Consults with supervisor regarding problems or concerns.
  • Other duties as assigned

Additional Job Functions:

  • Acts as Caseworker for UCAN (United Community Assistance Network) Program and EFSP for shelter residents as needed
  • Coordinates with other Neighborhood Alliance programs, such as child care or Help Me Grow, to support participants

Necessary Education and Training:

  • Licensed Social Worker preferred. Bachelor’s degree or higher in Social Work or Human Services is required
  • Must demonstrate good writing and verbal skills, computer skills a plus
  • Experience working with low income individuals and families preferred
  • Knowledge of Clarity, UCAN, EFSP and other community resources highly desired
  • Data Entry, Microsoft Excel and Word
  • Ability to remain calm and think clearly in a crisis is essential

Physical Demands: In order to perform the essential functions of this position successfully the employee may be required to sit, stand, reach, lift, bend, kneel, and stoop. This position requires manual dexterity, auditory and visual skills, and the ability to follow written or oral instructions and agency procedures. All functions must be able to be performed with or without reasonable accommodation.

Typical Working Conditions: This position operates in a homeless shelter/residential type environment. The employee may be exposed to loud noise and communicable diseases.

Work Site Guidelines: This position requires the employee to work in a variety of locations. A typical work week will include each of the following categories:

· In office: 60%

· Community: 40%

· Remote: 0%

Job Type: Full-time

Pay: $16.00 - $18.00 per hour

  • Case management: 2 years (Required)
