POSITION TITLE: Case Manager
POSITION OVERVIEW:
Neighborhood Connections’ Case Manager is a critical member of the organization delivering the day-to-day services to the populations and communities we serve. The Case Manager is responsible for client services case management and works directly with the Executive Director and other staff. Through initial intakes and assessments, the Case Manager will identify issues for assistance, develop goals and plans with the client, and coordinate referrals were appropriate.
The Case Manager is also responsible for crisis intervention, housing security and navigation, information and referral practices and collaboration with all available resource groups and follow-up on all cases until conclusion or closing.
POSITION RESPONSIBILITIES
Basic Intake functions, gathering all necessary data from clients upon entry to agency and utilizing intakes and applications appropriate to pressing client issues, including housing navigation programs such Coordinated Entry, VERAP, and Choices for Care applications, and providing support to complete applications.
The successful candidate will serve as the agency's point for contact for Coordinated Entry housing applications.
- Assessing needs and development of action plans with clients, including addressing medical and financial needs.
- Being knowledgeable of all public and private resources in the area available to the client.
- Assisting clients with application process for available services (i.e. healthcare, unemployment, 3SquaresVT, subsidized housing, assistive living facilities, Medicaid programs, social security, etc.)
· Providing logistical and other housing support to help housing insecure and at-risk households find and move into housing, such as developing an action plan for locating housing, identifying housing barriers, needs and exploring resources available to the applicant, addressing barriers to housing admissions providing outreach to and negotiation with landlords, and assisting in arranging for supporting moves.
- Informing client of accessible services based on eligibility requirements
- Advocating for the client with state and other agencies.
- Forming collaborative relationships with state and local agencies, including but not limited to Southeast Vermont Community Action (SEVCA), Springfield Supportive Housing, HCRS, Senior Solutions, and the state Division of Children and Family Services.
- Attending state and regional training sessions to remain current on application processes and service requirements and eligibility.
- Coordinating clients' appointments with medical professionals, social service agencies, and legal services, and utilizing the agency's in-house transportation system to provide transportation for those requiring it.
- Making home or onsite visits to assist those unable to come into the office.
- Reviewing client communications from state, regional, and federal agencies, and reviewing same with clients with a view towards revising or pursuing client action plans.
- Preparing correspondence for clients unable to do so on their own behalf.
· Providing ongoing follow-up support for those finding stable housing, such identifying and providing intervention for behaviors that may jeopardize housing, coaching on developing and maintaining key relationships with landlords/property managers to foster successful tenancy, and advocating with community resources to prevent eviction when housing may become jeopardized.
· Recording detailed case notes of client and provider interactions in a client database management system
QUALIFICATIONS:
- Bachelor's Degree in Counseling, Social Work, Human Services or related field required; Masters and Professional Social Work licensure preferred.
- 3 years experience with case management and/or care coordination or the equivalent
- Practical knowledge of state and federal program requirements, including HIPAA and FERPA requirements
- Excellent listening, problem solving and assessment skills
- Empathetic and compassionate with the ability to hold clear boundaries
- Respect for individual rights and choices
- Ability to form networks with other providers
- Excellent interpersonal skills
- Ability to multi-task and be organized (both in time and record-keeping)
- Valid driver's license with clean driving record and reliable transportation
- Basic understanding of Microsoft Office/Google programs
- Proven time-management skills
COMPENSATION:
This is a full-time position, consisting of a flexible four-day work week. Salary range is $48,000-$52,000 commensurate with experience. Employees receive 11 paid holidays, a paid week off between Dec 25 and Jan 1, and 120 PTO hours annually. While we are unable to offer health insurance, employees receive a stipend of up to $400/month for their health care costs. We participate in the Vermont Saves retirement plan.
