PRIMARY FUNCTIONS: 

The Cancer Screening Manager is responsible for overseeing and managing the cancer screening program within Neighborhood Health. This role involves coordinating screening activities, ensuring program efficiency, and maintaining high standards of patient care and safety. The Cancer Screening Manager will work closely with medical staff, administrative teams, and external partners to enhance screening processes and improve patient outcomes.

Incumbent must have a deep interest in cancer screening and navigation, and have a strong commitment to Neighborhood Health’s mission to improve health and advance health equity by providing high-quality primary care regardless of ability to pay.

DUTIES/ RESPONSIBILITIES: (Essential Functions: Duties and responsibilities noted with bullets are the essential functions of the job).  

Program Management and Supervision

  • Oversee cancer screening programs at Federally Qualified Community Health Center, focusing on early detection and prevention of breast, cervical and colon cancer.
  • Directly supervise cancer screening navigators and outreach workers
  • Train and oversee volunteers to assist with cancer screening program
  • Develop and implement outreach strategies to target high-risk populations and increase screening rates.
  • Monitor and evaluate program performance, ensuring adherence to best practices and regulatory requirements.
  • Recommend improvements to screening and navigation workflows to reach program targets
  • Provide training and professional development opportunities to ensure staff competency and compliance with program protocols.

 Data Management 

  • Maintain accurate records of screening activities, results, and patient demographics via patient databases and EHR.
  • Collect and document project data using EMR and population health tools, analyze data to identify trends and assess program effectiveness.  
  • Report findings to the Chief Medical Officer, Director of Community Partnerships, and Quality Manager and prepare grant reports for external stakeholders and funder.
  • Patient Navigation
    • Navigate individual patients through cancer screening and diagnostic evaluations, including linking patients to care with external partners and assisting patients with accessing appointments.
    • Perform escalated case management duties for vulnerable, or high-risk patients to include assistance with financial aid applications, specialty health care appointments, and prescription drug assistance.
    • Follow up with difficult-to-reach patients, assess reasons for declining care, identify patterns and barriers, and reschedule no-shows, as possible. 
    • Communicate with members of the patient’s Neighborhood Health care team on behalf of the patient.
    • Discuss patient findings (after provider review) with patient and explain next steps in patient care journey
    • Ensure a high standard of patient care and service, addressing patient concerns and providing support throughout the screening process.
    • Cancer Screening Outreach
    • Develop patient education materials and outreach programs to increase awareness and participation in cancer screening.
    • Run regular quality reports through the population health software for outreach to patients, reminders, and help assess the effectiveness of the quality program. 
      QUALIFICATION/EDUCATION and/or EXPERIENCE REQUIREMENTS:
       
       
      Required Experience
    • Bachelor’s degree in healthcare administration, nursing, public health, or a related field. Master’s degree or equivalent experience is preferred. 
    • Experience as a cancer screening manager or navigator preferred
    • Experience in program management or staff supervision preferred
    • Bilingual skills highly preferred (Spanish/English) 
    • Knowledge of cancer diagnostic evaluations 
    • Strong leadership and team management abilities.
    • Excellent organizational and project management skills.
    • Proficient in data analysis and reporting, including Microsoft Office, especially Excel.
    • Knowledge of healthcare regulations and quality standards related to cancer screening.
    • Strong organizational skills; attention to detail and accuracy
    • Good interpersonal, communication, and customer service skills
    • Compassionate and caring demeanor
    • Experience working with Electronic Medical Record system (training provided) 
    • Previous experience working with vulnerable communities, particularly the uninsured/Medicaid population 
    • Ability to work under minimal supervision with multiple internal and external partners collaboratively and supportively.
