ROLE OVERVIEW and PURPOSE

The Homecare Clinician will assess participants and their homes to determine appropriate interventions for the purpose of keeping participants safe at home. Interventions can include recommendations for home modifications, durable medical equipment, assistive devices, and homecare caregiver services.

RESPONSIBILITIES

  • Completes home safety assessments to determine if intervention is recommended to maximize participants' safety in their home.
  • Assesses the functional abilities of a participant to determine whether they may benefit from homecare caregiver services.
  • Works with members of the interdisciplinary team (IDT), participants, and families in developing the services, direction, and plans of care for participants.
  • Maximizes participant functional capacity by encouraging autonomy in all aspects of care.
  • Educates, supervises, and counsels participants and / or caregivers regarding home health care needs and other related problems.
  • Utilizes adult learning principles to plan and implement educational information for the participants, caregivers, or family members.
  • Initiates preventative and rehabilitative procedures or programs as appropriate for care and safety to participants.
  • Notifies appropriate medical personnel and IDT of changes in participant statuses.
  • Educates participants and families in safe administration of medication for home use, including assessing and encouraging compliance with medication regimen.
  • Recognizes and understands the significance of abnormal test results to assist in gathering data, planning, and implementing care.
  • Provides safe patient care to participants with complex health problems with a focus on individual participants and their families.
  • Leads and monitors licensed and other professional and nonprofessional staff in the delivery of home health care to participants in their homes.
  • Recommends and authorizes appropriate home modifications, durable medical equipment, and assistive devices in collaboration with the PACE rehab department's physical and occupational therapists.
  • Evaluates participant outcomes and progress toward achieving the objectives / goals of the care plan and communicates this information to IDT members.
  • Collaborates with IDT to revise the plan of care based on changes in participant physical or psychosocial status and initiates actions that are consistent with the changes in status.
  • Works with participants, families, and IDT members to evaluate / measure the individual and group response to home health care and teaching interventions, including documenting the outcomes of the problems identified at scheduled reviews.
  • Maintains accurate and timely records of participant’s functional health status, progress toward care plan outcomes, revisions to care plans, care given.
  • Advocates on behalf of participants and demonstrates accountability in resolving participant concerns or issues.
  • Complies and promotes the participant bill of rights, including assessing and working towards achieving high levels of participant satisfaction.
  • Provides clinical coverage in the PACE clinic, as needed (RN only)
  • Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government / regulatory agencies.
  • Enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
  • Attends meetings and participates on committees, as directed.

EDUCATION / EXPERIENCE

  • One year’s experience working with elderly, older adult populations required.
  • Valid CA driver license and proof of auto insurance required.
  • Bilingual (English / Spanish) preferred.
  • Valid BLS certification from approved American Health Association provider required upon hire.
  • For OT :
  • Graduate of an occupational therapy curriculum accredited by the Committee on Allied Health Education, American Medical Association, and American Occupational Therapy association required.

  • Valid CA occupational therapist license required
  • One-year occupational therapy experience; home or community health experience preferred.
  • For RN :
  • High school / GED required; bachelor’s degree in nursing preferred.

  • Valid CA registered nurse license required.
  • One-year registered nursing experience required; home or community health experience preferred.
  • ADDITIONAL QUALIFICATIONS (Knowledge, Skills, and Abilities)

  • Excellent verbal and written communication skills, including superior composition, typing, and proofreading skills.
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.
  • Knowledgeable about and experience with adult day health center or PACE programs
  • Ability to successfully manage multiple tasks simultaneously.
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently.
  • Ability to work with highly confidential information in a professional and ethical manner.
  • Physical Requirements

  • Ability to lift / carry 50 lbs. / weight.
  • Ability to stand for prolonged periods of time.
  • COMPLIANCE (Safety & HIPPA)

  • Follow all safety procedures as outlined in Neighborhood Healthcare’s Illness and Injury Prevention Plan (IIPP) and immediately report any injuries and / or unsafe conditions.
  • Maintains current knowledge of policies and procedures as they relate to safe work practices.
  • Follows all safety procedures and reports unsafe conditions.
  • Uses appropriate body mechanics to ensure an injury free environment.
  • Familiarity with location of nearest fire extinguisher and emergency exits
  • Follows all infection control procedures including blood-borne pathogen protocols.
  • Maintains privacy of all patients, employees and volunteer information and access such information only on a need-to-know basis for business purposes.
  • Comply with all regulations regarding corporate integrity and security obligations.
  • Reports all behavior and / or activity that are unethical, fraudulent, or unlawful.

    Pay range : $45.67-$67.78 per hour, depending on experience, education and additional qualifications.

    Compensation Disclosure :

    The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to : the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs.

    As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.

    Salary : $46 - $68
