Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.
As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 90k people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.
ROLE OVERVIEW and PURPOSE
The Personal Care Aide will support the CNA team by delivering personal care and companionship services to/from medical appointments with participants. This role will perform a variety of tasks throughout the PACE to ensure participants are provided services for activities of daily living. Additionally, this role will assist in the kitchen and during participant medical and therapy appointments.
RESPONSIBILITIES
- Provides direct personal care to participants, including showering, toileting, laundry, feeding assistance, companionship services, and other activities of daily living (ADL)
- Assists other staff in carrying out program assignments.
- Supports other PACE teams with program assignments for patients, including meal and snack services
- Assists in cleaning and organizing patient areas, including cleaning of equipment and shower areas
- Assists with home delivery of meals and medical supplies
- Follows infectious disease precautions to prevent the spread of organisms
- Attends and participates in center staff meetings, committees, and in-service educational training programs
- Communicates effectively with participants and co-workers
- Participates in interdisciplinary team meetings, as needed
- Adheres to established PACE, CMS, and ADHC policies and procedures
- Takes an active role in the Safety and Injury in the Workplace Program; maintains all records and documentation, as required
EDUCATION/EXPERIENCE
- High school diploma/GED required
- Six months’ personal care aide experience within professional/personal care setting required
- One year experience working within frail or elderly environment
ADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)
- Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
- Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
- Knowledgeable about and experience with providing care to others
- Ability to complete PACE approved curriculum for personal care aid
- Ability to successfully manage multiple tasks simultaneously
- Excellent planning and organizational ability
- Ability to work as part of a team as well as independently
- Ability to work with highly confidential information in a professional and ethical manner
Physical Requirements
- Ability to lift/carry 25 lbs/weight
- Ability to stand for long periods of time

*Pay range: $21.15 - $25.14 per hour, depending on experience/education, and additional qualifications.
