Job Summary:
The Behavioral Health Consultant is part of a multi-disciplinary team that delivers brief consultation-based services to community and primary care-based patients.
Duties/Responsibilities:

Provides full scope of consultative services including client screening and assessment, interventions (consistent with the integrated behavioral health model); facilitates classes and groups as well as social service support, referrals and/or resources.

Develops a measurable, time limited treatment plan.

Serves as resource for patients, families and healthcare employees.

Facilitates communication between family and other healthcare team members.

Collaborates with community resources to formulate and deliver a comprehensive multi-disciplinary treatment plan.

Effectively utilizes community resources to ensure that treatment plan goals are met.

Consistently meets position productivity expectations.

Participates in an integrated approach to patient care/treatment; work with team members to coordinate services that meet the goals and objectives of the health center.

Maintains appropriate clinical documentation in a timely manner.

Maintains quality care through participation in audit review, policy development and quality improvement programs.

Attends, participates, and/or assists in meetings, trainings, community outreach activities, continuing education opportunities, and other activities as required.

Performs other related duties as assigned.
Qualifications
Required Skills/Knowledge/Abilities:

Knowledge of behavioral health services and standards.

Excellent verbal and written communication skills and ability to interact with individuals of various social, cultural, and educational backgrounds.

Ability to speak English and Spanish preferred.
Education and Experience:
Required:
-
Master’s degree in Social Work, Counseling, or other behavioral health related field.

Actively licensed as: LCSW (Licensed Clinical Social Worker) or LPC (Licensed Professional Counselor) or LMFT (Licensed Marriage Family Therapist) or Licensed Psychologist.

License must be in good standing with Arizona Board of Behavioral Health Examiners.

1 years of providing behavioral health consultative services or other related experience.

3 years of healthcare experience.
Other Requirements:

New Hires are required to pass pre-employment background check and drug testing (effective 11/1/2022).

Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire.
