Neighborhood Outreach Access to Health (NOAH) is a non-profit Federally Qualified Health Center (FQHC) that provides comprehensive, integrated, and affordable high-quality healthcare services to underserved populations. Job Summary:
The Community Health Worker (CHW) works with patient care teams and the community to improve access to quality care by serving as a trusted liaison between the community and the organization.
Duties/Responsibilities:

Assists the Community Resource team, care coordinators, and navigators in the creation and implementation of individual care plans, with consideration to the social and cultural needs of the individual and family.

Provides culturally appropriate health and wellness training and education.

Reviews indicators of access and comprehensiveness of care with community resource team, care coordinators, and navigators to guide the work based on patient needs.

Facilitates access to services and improves the quality and cultural competence.

Builds individual and community capacity by increasing health knowledge through outreach, community education, informal counseling, social support, and advocacy.

Advocates for individuals and communities.

Acquires, submits, and facilitates paperwork for patients needing financial assistance with federal and community-based organizations.

Coordinates efficient and effective delivery of services.

Collects, enters, and maintains information in the Electronic Health Record System.

Attends, participates, and/or assists in meetings, trainings, community outreach activities, continuing education opportunities, and other activities as required.

Performs other related duties as assigned.
Qualifications
Required Skills/Knowledge/Abilities:

Knowledge of community health resources.

Understanding of general medical terminology and basic medical procedures.

Excellent verbal and written communication skills and ability to interact with individuals of various social, cultural, and educational backgrounds.

Education and Experience:

High school diploma or GED required.

Associates degree in health-related field preferred.

Community Health Worker training program completion preferred.

Basic Life Support (BLS) certification required.

1 years of customer service, healthcare, public health, or relevant work-related experience required.
Other Requirements:

New Hires are required to pass pre-employment background check and drug testing (effective 11/1/2022).

Must possess or be willing to obtain a current Community Health Worker certification within 1 month following hire.
-
Valid and current driver’s license and acceptable driving record.
-
Ability to travel to patients’ homes and other sites as needed.
