Position Summary:
Under the Supervision of the Lead Community Resource Navigator or assigned designee, the Housing Case Manager will be responsible for providing comprehensive case management services to individuals and families experiencing housing instability. The Housing Case Manager will assess participant needs, create individualized service plans, provide life skills training, and connect participants with community resources to promote housing retention and enhance overall stability.
Essential Duties & Responsibilities include the following:
- Conduct thorough assessments of participants’ housing situations, identifying immediate and long-term needs.
- Develop and implement individualized service plans that address the unique needs and goals of each participant.
- Provide ongoing support and case management services, including regular follow-ups and progress monitoring.
- Assist participants in navigating housing applications, benefits eligibility, and subsidy programs.
- Connect participants with community resources, including employment services, mental health support, substance use treatment, financial assistance, and legal aid.
- Provide life skills training to participants, focusing on tenancy skills, financial literacy, and safety planning to promote independence and self-sufficiency.
- Advocate on behalf of participants with landlords, housing authorities, and other relevant organizations to secure and maintain housing.
- Maintain accurate participant records in compliance with organizational policies and confidentiality standards.
- Collaborate with other staff members and community organizations to coordinate services and share resources.
- Utilize a housing-first, strengths-based, and trauma informed approach in all aspects of this position.
- Other duties as assigned.
Education and/or Experience:
Preferred: Associate Degree or higher
Required: High School Diploma or equivalent and 1 year of experience serving vulnerable and challenged individuals.
