What you will be doing:
- Assist Intake Coordinators with administrative tasks, ensuring efficient patient intake and scheduling.
- Accurately input and maintain patient data in Salesforce, Electronic Medical Records (EMR) systems, and logs.
- Conduct welcome calls to newly referred patients, providing an informative introduction to home health services.
- Communicate with referral sources, patients, and family members to gather missing or additional intake information.
- Support initial patient visit scheduling and coordinate with clinical teams to confirm appointments.
- Maintain organized records of patient referrals, admissions, and required documentation.
- Provide coverage in various intake-related areas as needed, adapting to workflow changes.
- Minimum of one year of administrative experience in a medical setting.
- Strong organizational skills and attention to detail with proficiency in EHR systems.
- Understanding of HIPAA regulations and the ability to handle confidential patient information.
- Excellent verbal and written communication skills for patient and provider interactions.
- Proficiency in Google Suite (Docs, Sheets, Gmail, Calendar) and other relevant software.
- Ability to work in a fast-paced environment with evolving deadlines while maintaining accuracy.
- Comfortable working in a dynamic setting with frequent interruptions and multitasking.
