This is a hybrid position, combining remote work with required travel within our 12-county service area. It is projected that most in-person work will occur in and around Otsego or Crawford County; however, candidates residing in any of the following counties are encouraged to apply: Alcona, Alpena, Arenac, Cheboygan, Crawford, Iosco, Montmorency, Ogemaw, Oscoda, Otsego, Presque Isle, and Roscommon. We have offices in Alpena, Cheboygan, and West Branch, and you will be assigned to the office closest to your location. The assigned office serves as your base, providing access to a desk and supplies as needed, but the position may allow you to primarily work from home. Candidates must be willing and able to travel as necessary for in-person components of the role.

What you will do:

Responsible for assessing individual’s needs, financial eligibility for multiple programs and services, including assisting participants with the Medicaid application process.  Assess each individual to determine what is important to them and complete a person-centered plan of care.  The Social Worker must analyze the environment of the at risk participant as well as arrange services for participants utilizing a person-center planning approach. Complete required forms; maintain participant files and related paperwork in a manner consistent with state, federal and agency guidelines and timeframes. Advocate for participants and their allies. Work as a team member in the development and implementation of care plans. Maintain ongoing communications with the participant, their allies and community entities. Ensure the health, safety and welfare of program participants.

Essential Duties and Responsibilities:   
  • Utilize strong interpersonal skills to work with participants, allies, agency staff, vendors, community and government entities;
  • Perform client assessments/reassessments and develop care plans utilizing person-centered planning guidelines;
  • Act as a strong client advocate;
  • When appropriate, assist participants with the completion of required Medicaid applications/verifications, follow-up to ensure timely eligibility determination and maintain communication with MDHHS local offices, participant and/or family and informal support systems;
  • Work with RN team member to determine participants’ medical eligibility for appropriate program enrollment;
  • Monitor financial eligibility and ensure appropriate funding mechanisms are utilized appropriately for client services at each assessment;
  • Assess participants’ living environments and when appropriate, assist in obtaining home modifications in accordance with policies;
  • Complete an emergency plan with the participant and their allies;
  • Comply with state, federal, and agency reporting requirements and mandated response time minimums;
  • Develop and coordinate comprehensive community-based service delivery in accordance with person-centered planning;
  • Participate in federal, state and agency meetings as required;
  • Assist with training or workshops and participate in program presentations as necessary;
  • Participate in educational and professional development as appropriate and obtain CEUs as required for licensure;
  • Represent agency at community meetings and participate in program presentations as necessary;
  • Establish and maintain working relationships with various outside county-wide entities such as MDHHS local offices, Community Mental Health, Public Health Department, Senior Center staff, hospitals, home health agencies, probate courts, legal providers, rural health clinics, long-term care facilities, and adult foster care homes;
  • Develop and maintain a list of countywide resources and programs, which may benefit a client in need of such services;
  • Participate in policy and procedure development as appropriate;
  • Maintain client confidentiality at all times;
  • Be knowledgeable of and prepared to follow appropriate procedures during an emergency situation in a client’s home;
  • Maintain professionalism while assisting participants or representing the agency at all times;
  • Prioritize tasks according to established criteria;
  • Required to report any suspected cases of abuse, neglect, or exploitation to the appropriate agency as well as the Community Based Care Supervisor;
  • Travel extensively throughout the multi-county service area.
