Nemours Children's Health seeks a highly skilled Nurse Navigator - Inpatient/Outpatient Transplant and Cellular Therapy to join our team in Wilmington, DE.
This role is responsible for coordinating high-quality care for patients requesting outpatient services at the Lisa Dean Moseley Foundation Institute for Cancer and Blood Disorders (LDMFI-CBD) and its sub-specialties.
The Inpatient/Outpatient Transplant and Cellular Therapy Nurse Navigator will serve as an advocate for complex patients, their families, and caregivers, helping them overcome healthcare system barriers.
Main Responsibilities:
- Ensure efficient patient care coordination between admissions, visits, scans, scheduling, and sub-specialty interfaces.
- Triage newly diagnosed patients and families with complex needs in inpatient and outpatient settings, providing procedure-related information during scheduling.
- Collaborate with physicians, healthcare teams, and patients/families to maintain a plan delivering quality healthcare in various settings, focusing on educational and resource needs.
- Provide guidance and education for highly complex patients and families to ensure necessary care throughout the care continuum.
- Coordinate and delegate prior authorizations and peer-to-peer reviews for complex patients.
- Collaborate with LDMFI-CBD specialty nurses for care coordination and cross-coverage as needed.
- Evaluate patient/family responses to care on an ongoing basis.
Requirements:
- BSN from an accredited program.
- Current, unrestricted DE Registered Nurse License required (subject to change based on patient base).
- Specialty Certification: CPON/CPHON certification preferred.
- Minimum 5 years of pediatric hematology/oncology nursing experience required.
