**Job Summary**
We are seeking a skilled physical therapist to join our team and provide high-quality patient care in the comfort of patients' homes.
The ideal candidate will have a Doctor of Physical Therapy (DPT) degree, current licensure to practice physical therapy in California, and at least 1 year of recent experience in an institutional setting or with a home health agency.
- Position: Home Health Physical Therapist
- Type: Full-time/Part-time, one weekend rotation per month
- Pay: Guaranteed base salary plus competitive per visit rates and performance incentives (For Full-time employees only)
**Key Responsibilities**
The physical therapist will be responsible for:
- Planning and administering prescribed skilled physical therapy treatment and training
- Testing and screening patient's physical strengths to assist in evaluating their level of function and recording findings
- Developing and implementing a conditioning/rehabilitation program consistent with the physician's Plan of Treatment
