Below are the general instructions regarding some of the onboarding forms:
- The New Employee Checklist - Please initial in front of all items applicable and put "N/A" in front of those items not applicable. Items 12-15 you will receive on the first day.
- Acknowledgement of Job Description Copy of Job Description linked in final offer letter.
- Uniform Advance and Allowance Information – A uniform advance/allowance is for Full-Time employees only, if the position states you will be required to wear a uniform, refer to the dress code in your final offer letter.
- Policy/Procedures acknowledgement Link to citywide policies on form
- W-4 Form - Please include your middle initial in the Step 1 (a) "First name and middle initial" field, and input complete information of City, State and Zip Code under Step 1 (a) "City of town, state and ZIP code" field.
- Direct Deposit - Direct deposit is required, and you may designate up to three banks, please ensure that account number and routing numbers that you list are correct. The City of Henderson does not issue paper checks.
- EEO Form - This is for the State of Nevada, and Federal report.
- Employees' Association Sign-Up Form & Letter - (For Full-Time only) Please note that the enrollment to HCEA is voluntary with a $10/mo membership fee and you can sign up or terminate the membership anytime in the future as well.
- Fingerprint Background Waiver - Form was completed during pre-hire assignments if required.
- Notification of Health Marketplace - (For Part-Time only) Mark as "N/A" if Full-Time.
- PD Fingerprint Unit Form - Complete bottom portion with your personal information.
- Workers' Compensation Informational Wallet Card - HR staff will issue you a hard copy card during onboarding.
- DuPont Safety/Citywide Online Training -The safety and compliance training courses will be assigned within a few weeks after onboarding based on the classifications.
- The New Employee Information Takeaways – This packet will be provided on your first day.
- The PERS Retiree Form – (Frequently Asked Question): Am I a PERS retiree? If you are not receiving the monthly payment from Nevada PERS, then you are not a PERS retiree and will not need to complete the PERS Retiree Form, you can choose skip on the form to complete the task.
- PERS Survivor Beneficiary Designation Form A. Under Nevada law, if a member is married, their spouse is automatically their PERS Survivor Beneficiary. A Survivor Beneficiary is the recipient of a lifetime death benefit in the event of an eligible PERS member's death. Members may want to consider listing at least one person in the Survivor Beneficiary section of the form (not a spouse, trust or charitable organization) to receive a lifetime benefit in the event of the spouse's death or member and spouse's simultaneous death prior to retirement. You may list children in the Survivor Beneficiary section even if you have listed them under the Family Beneficiary section. If you select a minor child/children as your Survivor Beneficiary/Additional Payee they may be eligible to receive a limited benefit until age 18 (or longer if they are a full-time student), in addition to a lifetime survivor benefit that will continue for the lifetime of the first person listed in the Survivor Beneficiary section. Once a survivor benefit has begun, additional payees would stop receiving the benefit in the event of the death of the survivor beneficiary.B. If you do not want to list a survivor beneficiary, please input "None".C. Important! All beneficiaries' DOBs are required.D. A Tertiary Beneficiary designation is only applicable if you've ever paid into your existing PERS account on an Employee/Employer contribution plan. If you've never been enrolled into PERS , this is not applicable to you at this time. If you have do have a PERS account where you participated in an Employee/Employer contribution plan then please complete the tertiary for any funds that you contributed in the event that there is no eligible spouse, survivor beneficiary, or minor children benefits to be paid. In this case, a tertiary beneficiary would be eligible to receive a one-time lump-sum payment of any existing member contributions in the System. They would not receive a lifetime survivor benefit.
- I-9 Employment Eligibility Verification Form Please complete the form by listing your name exactly the same as it shows on your social security card. (If there is a suffix i.e. "I, II, III, Jr, Sr" on your social security card, please INCLUDE it in your last name on I-9 form). If you have a middle initial, do not put a "." after it.A. All the required areas must be completed with accurate personal information with regards to your full name, other legal names used, address, date of birth, social security number, email address and phone number, or N/A if not applicable on the I-9 form.B. On your first day you must bring the original photo ID and/or appropriate combination of I-9 materials (passport, driver's license, State-issued identification card, permanent resident card, etc.) (no photocopies) with you to onboarding on your first day. There are no exceptions to this requirement.C. Upload copies of the I-9 verification Documents to allow HR to review. (If submitting a passport, the entire barcode page at the back of the passport must also be uploaded in order to be used for your I-9 verification.
