What is the purpose of this role?
The License Clinical Social Worker supports the holistic healthcare approach by providing behavioral health services. This allows Nest to expand the care we can provide for our families and ensure they have the care they need in all areas of their health.
What is the role?
The Nest Licensed Clinical Social Worker (LCSW) is responsible for supporting Nest members of all ages (children, adolescents, adults) with behavioral health services by providing initial evaluations, individual, group, and / or family therapy using telehealth modalities and connection to long term therapy resources. The LCSW will provide behavioral health consultation services to the Nest primary care team and have access to a consulting psychiatrist. The LCSW will work as part of an interdisciplinary team that includes medical assistants and advanced practice providers who go into member homes, and physicians, nurses, and dieticians who interact with members via telehealth modalities.

What will you do?
- Initial mental health evaluations resulting in realistic individualized treatment plans
- Complete assessments using standardized assessment / screening tools and measures
- Individual and family psychotherapy with children, adults, and families through virtual care modalities
- Provide brief, solution focused therapy in compliance with internal policies, external governing statuses and guidelines including the Department of Children and Family Services and other pertinent institutions
- Crisis assessments / triage with appropriate interventions
- Provide health education to members, care team, and practitioners within area of specialty, and other services related to wellness
- Review caseloads regularly to ensure appropriate utilization of services
- Attend team huddles and case conferences to advice on behavioral health needs and establish health goals as part of member's NAP (Nest Action Plan)
- Complete written documentation for all services rendered to members in Nest Health's electronic medical record (EMR) ensuring records are accurate, complete, client specific, and completed within specified timeframes
