Location: Catalina Island Medical Center, CA, US
Duration: Permanent, Full-time
Schedule: Mon - Fri, (8*5) with availability for at least two Saturdays a month depending on admissions
Guaranteed Hours: 40 hrs./week
Key Duties & Responsibilities:
Certification/Licensure/Experience:

Key Duties & Responsibilities:
- Coordinate and manage swing bed admissions, ensuring all admission requirements, screens, and tests are met within appropriate timeframes.
- Review emergency department admissions and discharges and follow up with patients to coordinate care with primary care providers (PCP) or other facilities as needed.
- Provide daily case management, social services, utilization review, and discharge planning, including weekends as scheduled.
- Perform financial assessments to determine patients' payment sources and initiate timely payment processes for services rendered.
- Develop and maintain relationships with specialty offices to facilitate patient transfers and collaboration of care.
- Participate in interdisciplinary team meetings and attend utilization review (UR) sessions.
- Assess and coordinate community resource interventions based on patient needs.
- Respond to queries from doctors' offices, families, and external facilities regarding patient care and discharge plans.
- Educate patients and families about the disease process, treatment options, and ongoing care requirements.
- Bilingual in Spanish is preferred.
- Ability to multitask and manage competing priorities in a fast-paced environment.
- Proficiency with computers and various software systems, including Microsoft Office, Athena, EMR, and Point Click Care.
- Strong problem-solving skills, with the ability to identify, analyze, and resolve patient care issues effectively.
- Excellent communication and interpersonal skills, with the ability to connect and collaborate with various departments and external partners.
Certification/Licensure/Experience:
- Current licensure to practice as a registered nurse in the state of California.
- Three to five years of experience in clinical nursing.
- One to two years of experience in case management or a related field is preferred.
- Bachelor’s degree in nursing is highly preferred.
- Basic Life Support (BLS) certification is required; Advanced Cardiovascular Life Support (ACLS) certification is preferred.
- Availability to work at least two Saturdays a month is required.
- Ability to manage multiple tasks effectively in a dynamic environment.
- Must possess a positive attitude, self-motivation, and the ability to work autonomously and as part of a team.
Salary : $69 - $76
