Job Summary

We are seeking an experienced Manager of Case Management to oversee the day-to-day operations of the case management team at a well-established healthcare facility. This leadership role is responsible for ensuring effective care coordination, optimizing patient throughput, and maintaining compliance with hospital policies and regulations.

Key Responsibilities

• Supervise and mentor the case management team, ensuring high-quality care coordination.
• Collaborate with physicians, nurses, and other healthcare professionals to facilitate patient-centered care.
• Oversee staff scheduling, training, and performance evaluations.
• Monitor documentation, case assessments, and adherence to care plans.
• Support initiatives that improve patient outcomes while optimizing resource utilization.
• Ensure compliance with regulatory requirements and hospital policies.
• Serve as a resource and advocate for patients, ensuring their needs and preferences are considered.
  
  

Qualifications

• Active RN license (Missouri or Compact).
• Preferred: At least 1 year of Case Management experience (acute care experience not required).
• Considered: RN leaders with experience as a Charge Nurse, Supervisor, etc., even without direct Case Management experience.
• Strong leadership, communication, and problem-solving skills.
