For Wyman specifically, CSW must provide integrated, interdisciplinary geriatric assessment and care. Must have a passion for working with elders who may be more vulnerable as a consequence of advanced age, or medical comorbidities and issues related to aging. The ability to develop trusting relationship with diverse patients struggling with mental health and/or substance use disorders. The ability to develop trusting relationships with family members is an essential component of the position.

Job Requirements
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
- Masters degree in social work from an accredited school.
- Minimum 2- years of experience in health care, with special interest and understanding of mental health diagnosis, substance issues, and interpersonal/domestic abuse.
- LCSW licensure with plan for LICSW.
- Excellent interpersonal and organizational skills. Strong computer skills. Demonstrated ability to work collaboratively in interdisciplinary teams.
- Ability to exercise sound clinical judgement in fast-paced and demanding settings.

- Perform biopsychosocial evaluations in accordance with departmental policies and standards.
- Document notes related to ongoing work in a timely and comprehensive manner.
- Collaborates with MD in planning and participating in family meetings.
- Provides support and education to patients and families related to illness, hospitalization, and post discharge options and follow up.
- Assists in facilitating the continuum of care and accessing community services for patients and families as needed.
- Completes insurance reviews as indicated.
- Utilizes supervision by bringing appropriate materials for discussion and following up on recommendations.
- Participates in daily rounds and consults with other clinical staff to facilitate enhanced patient care.
