Community Well-Being Ambassador - New Castle County
A Community Well-Being Ambassador is responsible for connecting members of the local community to resources, programming and well-being skills. In this position, your duties include being a part of a community outreach team, connecting with community members, and developing positive relationships with the community at large.
Ambassadors may operate as an assigned team member of a community organization or as a single ambassador at large.
Ongoing paid training will be provided. Ambassadors will serve grassroots organizations, where they will assist in capacity building through improved and expanded services. In addition, each Ambassador will participate in on-going training in areas such as leadership,
trauma-informed care, community engagement and more!
Responsibilities:
- Establish positive, supportive relationships with communing members and provide feedback
- Help community members in utilizing resources, including scheduling appointments, and assisting with
completion of applications for programs for which they may be eligible
- Assist community members in accessing well being related services
- Facilitate communication and coordinate services between providers and partners
- Motivate community members to be active, engaged participants in their health
- Attend all required training and refreshers
- Attend community meetings and events in their assigned zip codes.
- Effectively work with people from diverse backgrounds in reducing cultural and socio-economic barriers
between community members and institutions
- Build and maintain positive working relationships with the community members, providers, nurse case managers,
agency representatives, supervisors and office staff
- Continuously expand knowledge and understanding of community resources, services and programs provided;
human relations and the procedures used in dealing with the public as part of a service or program; volunteer resourcesand the practices associated with using volunteers, operations, functions, policies and procedures associated with thedepartment or program area, procedures and resources available to handle new, unusual or different situations
- Identify and apply skilled boundaries
- Other duties as assigned

- Any combination of 3 years health/social services experience and/or education
- Bilingual/bicultural (Spanish) encouraged to apply
Job Type: Full-time
Pay: $35,000.00 - $40,000.00 per year
