Responsibilities:
Carefully review physician orders, assess medication appropriateness based on patient’s medical history,
rehabilitation plan and recommend adjustments as needed.
Provide comprehensive medication counseling to patients and caregivers, explaining drug names, dosages,
side effects and proper administration techniques.
Monitor patients’ response to medication therapy, including regular review of laboratory values and
clinical parameters and identify potential adverse drug reactions.
Accurately dispense medications, maintain proper inventory levels and order medications as needed,
adhering to all pharmacy regulations.
Actively participate in team rounds, collaborating with physicians, nurses, therapists and other healthcare
providers to discuss medication management strategies and optimize patient care
Qualifications:
- Doctor of Pharmacy Degree
- Current Louisiana Pharmacy License
- A minimum of two years or more pharmacy experience in a hospital setting
