So, with that being said, our next position we need to hire right away is for a Director of Case Management and Care Coordination.
We need a licensed therapist, or a RN with 3 or more years of BH experience.

Overview
As the Director of Case Management & Care Coordination at NovumHealth, your primary purpose is to oversee and lead a team responsible for coordinating patient care services, ensuring seamless transitions and continuity of care from a therapist's perspective. You will play a crucial role in optimizing care coordination processes, collaborating with healthcare professionals, and promoting the delivery of high-quality, patient-centered care.
Tasks include, but are not limited to:
- Develop a local program based off of extensive corporate experience from our other markets.
- Hire and train your own team!
- Oversee the day-to-day operations of the care coordination team, providing guidance, support, and leadership.
- Develop and implement care coordination protocols, policies, and procedures to ensure efficient and effective coordination of patient care.
- Collaborate with healthcare providers and interdisciplinary teams to facilitate timely and appropriate patient referrals, consultations, and transfers.
- Monitor and evaluate the effectiveness of care coordination processes, identifying areas for improvement and implementing strategies to enhance patient outcomes.
- Ensure compliance with regulatory requirements and quality standards related to care coordination.
- Foster a culture of collaboration, communication, and teamwork among care coordination staff and other healthcare professionals.
- Provide training, education, and mentorship to care coordination team members to enhance their skills and professional growth.
- Participate in strategic planning and initiatives to enhance care coordination and patient experience.
- Additional Duties as assigned.
Skillset
- Strong leadership and management skills.
- Knowledge of care coordination processes and best practices in SLC.
- Understanding of healthcare regulations and quality standards in SLC/Utah.
- Excellent communication and interpersonal skills.
- Ability to collaborate and work effectively with interdisciplinary teams.

- Proficiency in healthcare information systems and technology.

Education and Experience
Years of Experience
Minimum of 5 years of Behavioral Healthcare, with at least 2 years in a care coordination or management role
Must have extensive knowledge of local behavioral health delivery systems and social service agencies.
Must be a fully licensed therapist in the state of Utah.
Master's degree required.
