Job Summary:
We are seeking a dedicated LPN who has experience in home care assessment to join our support team. This nurse assessor will be responsible for doing in home nurse assessment and working along side the RN coordinators to ensure compliance with Virginia Department of Health and DMAS regulations. They will perform non-skilled home care visits and providing clinical support to home care agencies as needed.
Responsibilities:
- Coordinate patient care activities and collaborate with healthcare providers
- Develop and implement patient care plans using DMAS forms
- Monitor patient progress and outcomes
- Provide education to patients and families on treatment plans and available benefits
- Maintain accurate medical records and documentation
- Ensure adherence to regulatory standards and protocols
Requirements:
- Associate Degree in Nursing or Bachelor's degree in Nursing (BSN) required;
- Valid LPN license in the state of Virginia or a compact license
- Minimum of 1 year of experience in a clinical setting
- Proficiency forms such as DMAS 99/97ab/100/7a and ICD-10 coding
- Strong knowledge of nursing documentation
- Ability to work effectively communicate and interact with clients in their home
