Outpatient Case Manager (Bilingual Spanish)
New Alternatives, Inc. is a private nonprofit agency that has provided a wide range of services to children, youth and families in San Diego and Orange Counties since 1978. Therapeutic services include residential, outpatient, day treatment, crisis support and in-home support services.

North County Outpatient School Based Services is contracted with both the County of San Diego and San Marcos Unified School District to service youth in North County. Our focus is to provide school-based therapy to clients who are uninsured, low income, or have full scope Medi-Cal with behavioral health needs.
At New Alternatives, Inc., we believe that challenging behaviors are the direct result of individuals lacking the developmental skills needed to manage day to day demands. We believe that these children are doing the best that they can given the resources and circumstances at that given time. We utilize the power of relationship with our clients to develop lagging skills, as well as evidenced based interventions in our therapy sessions.
SERVICES
We provide a variety of Behavioral Health services and supports to the youth and families we service which could include: screening, assessment; individual and group treatment; consultation and training for school personnel, parents, caregivers, and other collaterals; maintaining a written record of treatment, including initial assessments and treatment plans; individual, group and family therapy.
MINIMUM REQUIREMENTS
- Bachelor's Degree in Psychology, Social Work, Sociology or a closely related field from an accredited university
- Previous experience working with youth and families with mental and behavioral health needs
- Able to work with multi-disciplinary team in a very diverse setting that serves people of all backgrounds, cultures and sexual orientation and willingness to learn more.
- Strong professional oral and written communication skills is a must, as well as, ability to work with minimum supervision and frequent interruptions, while maintaining a professional demeanor at all times.
- Must be computer literate.
PREFERRED QUALIFICATIONS
- Bilingual Spanish - strongly preferred.
- Strong collaboration and communication skills with the ability to work both
- independently and as a team.
- Knowledge of Trauma Informed Care, Positive Discipline, Positive Behavioral Supports, Behavioral Modification/Interventions, Motivational Interviewing, Parenting skills, and/or willingness to learn
- Ability to conceptualize a client from a wide lens, taking into consideration the trauma, early childhood experiences, culture, family dynamics, physical health, social functioning, intellectual ability and other domains that contribute to the client's current presentation
- Experience providing individual and family support to adolescents and adults (ie.caregivers) and be creative in engaging families to meet them where they are at.
- Case Management skills and experience with linking families and clients to supports in their community
- Ability to research and find the resources to meet family’s needs.
- Familiarity with the Substance Use, Mental Health, Juvenile Justice/Probation and Social Services systems preferred.
ESSENTIAL DUTIES AND RESPONSIBILITIES (Other duties may be assigned)
- Provides individual, family and group services in a school/office based setting.
- Facilitates individual and group skills-building activities to help clients learn coping strategies, emotional regulation, and daily living skills.
- Develop individualized rehabilitation/case management plans tailored to clients' specific needs and goals. Provide ongoing support and encouragement throughout the treatment process.
- Facilitates linkages and provides education regarding community resources to better support the youth and family with needs that will support their treatment outcomes.
- Collaborates and monitors progress of assigned cases and stays in close communication with clinician, school, parents and community partners regarding improving social, emotional, and behavioral functioning.
- Maintain accurate, complete, and timely case records and client charts in Electronic Health Record, SmartCare.
- Maintains positive relationships with school staff, co-workers, clients and families.
- Required to travel between office and schools, in personal vehicle
- Active participation in supervision, team meetings, and team collaboration meetings
- Willingness to gain additional knowledge and understanding to work interdisciplinary approaches and work alongside in partnerships
- Perform any other duties as directed
