Job Overview
The Mental Health Case Manager plays a crucial role in supporting individuals facing mental health and Substance Use challenges. This position involves working collaboratively with clients to assess their needs, develop treatment plans, and connect them with appropriate resources. The ideal candidate will have experience in various care settings, including acute care, managed care, and outpatient services. This role requires a compassionate approach to patient care and a commitment to improving the mental health outcomes of those served.

Duties

  • Conduct comprehensive patient/client assessments to determine mental health needs and develop individualized care plans.
  • Coordinate and facilitate access to necessary services, including inpatient and outpatient treatment options.
  • Collaborate with healthcare providers, social workers, and community resources to ensure holistic patient support.
  • Monitor patient progress and adjust care plans as needed based on ongoing assessments.
  • Provide education and support to patients and their families regarding mental health conditions and treatment options.
  • Document all interactions and maintain accurate records in compliance with healthcare regulations.
  • Participate in case reviews and team meetings to discuss patient progress and share insights.

Skills

  • Strong understanding of mental health principles, including acute care, managed care, and addiction counseling.
  • Experience working in inpatient settings or Level I Trauma Centers is preferred.
  • Proficient in patient assessment techniques and developing effective treatment plans.
  • Excellent communication skills for interacting with patients, families, and multidisciplinary teams.
  • Ability to work effectively in high-pressure environments such as emergency medicine situations.
  • Compassionate approach to patient care with a focus on empathy and support.
  • Strong organizational skills to manage multiple cases effectively while maintaining attention to detail.
