Job Description:
New Breed Creation Life Center, Inc. a non-profit community-based organization, is recruiting for an experienced Prevention Navigator/Case Manager to provide high level care coordination to clients in Project Hope. This position will also have responsibility to provide Substance Use and HIV Prevention Services to racial and ethnic minority populations at high-risk for substance use disorders and HIV Infection.
The Prevention Navigator/Case Manager is also responsible for ensuring that the data collection activities for all clients at program entry, reassessment and follow-up and discharge. This position requires extensive community interaction and outreach. Must possess the ability to motivate participation in the program, motivational skills and influence to gain program participation.

Responsibilities and Duties:
· Work as a member of an interdisciplinary team of mental health and medical
professionals.
· Provide comprehensive case management services to clients. Thorough knowledge of community resources for linkage and referral.
· Maintain community presence through regular marketing and outreach.
· Knowledge of computer-based data management programs and information systems.
· Maintain accurate clinical records and documentation.
· Diligently meet grant metrics and treatment guidelines.
· Collaborate and assist medical team with programmatic interventions for the education of patients and their families to manage and promote healthcare compliance and wellness.
Qualifications:
· A bachelor’s degree or master’s degree from an accredited graduate program in the
human services field, And a Certification as a Case Manager through the Florida Certification
Board is preferred.
· A minimum of two years of experience providing a full range of case management
services to the target population.
· Knowledge of risk assessment and crisis intervention.
· Ability to successfully pass Level Three background screening required.
· Knowledge regarding mental health disorders, substance use disorders and co-occurring disorders. Familiarity with DSM 5 diagnostic criteria.
· Detail oriented and ability to manage a diverse caseload while meeting grant metrics.
· Excellent organizational and interpersonal skills as well as training and public speaking
experience. Establishing and maintaining effective relationships with all stakeholders.
