Dakota Conservators Inc. provides Guardianship, Conservatorship, and Trustee services to individuals in the Twin Cities area. We have over 25 years of experience and are dedicated to providing exceptional services to individuals with mental health diagnoses, memory loss, and developmental disabilities.
GENERAL JOB DESCRIPTION
Dakota Conservator's is currently seeking Part-Time Guardian/Conservator to provide case coordination to appointed vulnerable adults.
MAJOR DUTIES AND RESPONSIBILITES
· Responsible for case coordination
· Providing advocacy and being a liaison in the metro area to those served
· Ability to work independently and within a team setting
· Communicate professionally, written and verbally
· Participate in Judicial matters
· Complete all required documentation in a timely matter
· Demonstrated ability to communicate effectively with a variety of diverse individuals
MINOR DUTIES AND RESPONSIBILITIES
· Travel to homes, hospitals, and facilities where individuals are served
· Knowledge of rights of individuals served or the ability to learn
QUALIFICATIONS FOR THE JOB
Education:
· BA in Social Service field, preferred
Experience:
· 5 years verifiable experience in the human services field in lieu of a degree
· Knowledge of social work principles
· Knowledge of MS Office applications
Other:
· Valid driver's license and insurance
· Ability to effectively communicate verbally and written in English
· Availability during weekdays
· Ability to be available for on-call emergencies
