We are looking for a compassionate and self-reliant Licensed Vocational Nurse (LVN) , who has experience in or who would like to grow into the area of Home Health Care.
As a Home Health Care LVN, you will apply your experience and knowledge to the needs of your clients, allowing them to remain within the familiar surroundings of their own homes.

Responsibilities
Follow the plan of care assessment done by RN and approved by the MD.
Collaborate effectively with the patient’s healthcare team to establish an optimal discharge plan. The healthcare team includes physicians, referral coordinators, discharge planners, social workers, physical therapists, etc.
Identify specific patient needs, assure that the patient is progressing toward discharge goals, and assist in resolving barriers.
Assess signs and symptoms indicating physiologic changes in the patient’s condition. Continue to monitor Patients conditions and perform appropriate skilled service. (indicated in the plan of care or physician orders)
Collect, analyze, and interpret data and information from health care members in order to document actual and / or potential nursing diagnoses.
Other Licensed Vocational Nurse (LVN) duties as assigned.
Enter timely and accurate documentation into the computer.
Qualifications
Current Licensed Vocational Nurse (LVN) License in the state in which the Licensed Vocational Nurse (LVN) practices.
Ability to travel to multiple patients per day
One year prior Licensed Vocational Nurse (LVN) experience preferred.
