Care Coordinator for Adult Outpatient and Day Treatment Programs
Position Overview: New Day, Inc. is seeking a dedicated Care Coordinator to join our Adult Outpatient and Day Treatment programs. The ideal candidate will ensure compliance with state statutes, COA standards, and federal regulations, including HIPAA and 42 CFR Part 2. This role involves collaboration with Counselors and Peer Support Specialists, working as a vital member of the treatment team to coordinate care for clients referred to our Adult Services.
Key Responsibilities:
- Ensure adherence to state and federal regulations and organizational standards.
- Assist Counselors and Peer Support Specialists in delivering services.
- Represent New Day, Inc. positively in the community, promoting our mission.
- Coordinate care for clients, helping them identify and access community resources and services.
- Maintain accurate records and documentation of client interactions and progress.
- Facilitate educational groups for clients to enhance their understanding and skills.
- Provide transportation for clients as needed.
Qualifications:
- Bachelor's Degree in Human Services, Social Work, or a related field, or equivalent experience is required.
- At least one year of experience in the Mental Health or Substance Use Disorder (SUD) field is preferred.
- Ability to take direction and work collaboratively with the treatment team.
- Empathy and understanding of addiction issues.
- Capable of implementing positive reinforcement strategies with clients.
- Strong accountability for problem resolution, meeting goals, and serving clients, the team, and the organization.
Compensation:
- Wage: $23.00 per hour.
- This is an in-person position.
