Direct Care Worker Job Description
Hours: Varies (full time and contractual also available)
The Direct Care Worker (DCW) under the supervision of the Rehabilitation Specialist will assure continuity of care for clients in the Psychiatric Rehabilitation Program and perform related duties as required.
- Attend at least 60 hours of staff training in the area of psychiatric rehabilitation prior to providing any direct services. In addition, DSW will participate in ongoing quarterly trainings to equal at least 10 hours per year in the area of psychiatric rehabilitation.
- Participate in the initial assessment and development of the individual rehabilitation plan (IRP) with guidance from the Rehabilitation Specialist.
- Responsible for implementing the rehabilitation activities outlined in the individual’s treatment plan.
- Provide assistance to client in the area of: self-care skills, social skills, independent living skills, cultural interests, medication monitoring, health promotion, individual wellness self-management and recovery.
- Provide assistance with concrete tasks such as getting driver’s license or state ID, gaining employment, finding stable housing and apply for appropriate entitlement programs.
- Complete client assessments and IRP paperwork with assistance from Rehabilitation Specialist .
- Complete session note for each client visit as well as monthly progress notes with assistance from Rehabilitation Specialist.
- Turn in all necessary paperwork by given deadlines.
- Participate in ongoing on-site and off-site sessions with client with a minimum of 3 sessions per month for minors and 6 sessions per month for adults.
- Observe and report to Rehabilitation Specialist observations of client’s behavior and progress towards goals.
- Collaborate with referring therapist and other treatment team members to ensure the client’s needs are being met.
Qualifications:
Have at a minimum, a high school diploma or GED and 1 year of work experience in a supervised mental health setting or 30 hours, or their equivalent, of college credit toward a bachelor's degree in a health-related field; candidate will also have the ability to read, understand and follow detailed departmental procedures and state and federal laws and regulations; learn practices of care management including evaluating needs of clients, referring clients to resources, and providing skill development for clients. regulations; learn practices of care management including evaluating needs of clients, referring clients to resources, and providing skill development for clients.
