Case Manager
New England Medical Group provides integrative health care and wellness services for Mental Health and Addiction disorders. Our holistic approach seeks to bridge the gaps in services for people seeking medical and therapeutic support for life in recovery. We are currently seeking an experienced Case Manager with a passion for helping people for a mental health treatment facility at our Milford, MA location.
Position Overview: The Case Manager works closely with the clinical team to support and streamline the needs of the program at New England Medical Group. They will be involved in all pre-admission and potential client follow up, and support the clinical department through assessing client needs, communicating client’s treatment options, coordinating care and gauging progress. They will support compliance with fiscal, ethical and legal standards and the core values and mission of New England Medical Group. This is a full-time in person Monday-Friday position.
Specific Responsibilities:
- Act as the patient's advocate as it relates to insurance coverage, financial assistance, state/employer benefits, and accessing available resources.
- Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation.
- Communicate patient needs to a variety of care team members and follow up accordingly.
- Manage continuing care plans throughout the course of treatment, provide appropriate referrals.
- Collaborate with our CR (Community Relations) team for referrals and aftercare.
- Work collaboratively with clients, families, and clinical team to ensure high quality care.
- Maintain the patient's comprehensive clinical record through detailed documentation.
- Facilitate group therapy sessions
- Coordinate with third party contacts on a regular basis.
- Effectively communicate our services and programs, information about our specific cohorts to clients.
- Complete all front end set up for clients on Electronic Medical Record (EMR).
- Initiate follow up and follow through without direction, reach out for support when needed.
- Gather and confirm all admissions/financial documents are signed off uploaded in EMR.
- Gather and confirm all admission documentation has been signed by client including their Emergency Contact (EC) and uploaded into EMR.
- Develop and maintain a working knowledge of company protocols and procedures specific to Intake/Admission.
- Coordinate client referrals with team as needed.
- Documenting all interaction/attempted outreach in Case Management notes on EMR.
- Sending client satisfaction survey to all discharging client.
- Attend all team meetings as required.
Minimum Qualifications
- Bachelor’s Degree Preferred (not required)
- 1 years of experience in a related field at any level of care (RTC, CSS, IOP, or OP).
- Knowledge and familiarity with PHP/IOP/OP best practices and ways of working required.
- Ability to work with an Electronic Medical Record (EMR) and to assist patients with state/employer benefits (such as Short-Term Disability and Paid Family Medical Leave).
- Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.
- Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources and all other contacts.
