We are seeking a compassionate and driven Housing Case Manager to join our team and help us fulfill our mission.
Job Summary:
The Housing Case Manager will serve as a vital resource for clients, providing them with assistance and support services.
Key Responsibilities:
You will be responsible for driving clients to and from the clinic and to outside group activities as directed. You will be responsible for ensuring the cleanliness and order of your house. You will be responsible for assisting clients with medication management, scheduling appointments and ensuring that all house rules are followed as directed.
Advocacy and Support:

- Advocate for the needs and rights of clients in housing matters.
- Provide emotional support and motivate clients in their pursuit of stable housing and self-sufficiency.

Crisis Intervention:

- Respond effectively to housing crises and emergencies, providing immediate support and intervention as needed.
- Collaborate with local services to ensure client safety and well-being.

Documentation and Reporting:

- Maintain accurate and confidential client records, documenting services provided, progress towards goals, and any changes in clients’ circumstances.
Collaboration:
*
- Work closely with other staff to coordinate services and share resources.
- Attend regular team meetings and collaborate on client cases to ensure comprehensive care.
- Qualifications:
Proven experience in case management, social services, or housing support.

Strong interpersonal skills, with the ability to build rapport and trust with diverse populations.
- Excellent organizational and time management skills, with the ability to manage a caseload effectively.
- Proficiency in using computer software for case management and reporting (e.g., Google Suite, case management software).
- Ability to work independently as well as part of a collaborative team.
- Strong problem-solving and crisis intervention skills.
- Commitment to anti-discrimination and social justice principles.
Preferred Qualifications:
- Experience working with specific populations (e.g.,individuals experiencing homelessness, with substance use disorder, and mental health conditions).
- Bilingual or multilingual abilities are a plus.
Working Conditions:
- This position may require travel within the community to meet with clients and resources.
- Flexibility in hours may be required to accommodate client schedules and emergencies.
Job Type: Full-time
Pay: $20.00 per hour
Expected hours: 40 per week
Schedule:
- 8 hour shift
- Every weekend
- Nights as needed
