The State of New Hampshire, Department of Health & Human Services, Healthcare Facilities, Glencliff Home have part-time vacancy for a Licensed Nursing Assistant I-II.

Summary:

LNA I

Assists and delivers direct care to psychiatric and/or developmentally delayed residents to whom they are assigned. Assists residents with Activities of Daily Living (ADL) and performing related tasks to provide a safe, clean environment.

LNA II

Assists and mentors Nursing Assistant I’s in providing individualized direct care to psychiatric and/or developmentally delayed residents, including the care and maintenance of sanitary conditions of residents and surroundings: Works under the general direction of a licensed nurse.

YOUR EXPERIENCE COUNTS : Each additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education. MINIMUM QUALIFICATIONS:

LNA I

Education: High school diploma or high school equivalency credential.

Experience: No experience required.

License/Certification: Must possess and maintain a current license for Nursing Assistant as approved by the New Hampshire Board of Nursing. Must possess a valid driver’s license if responsible to transport residents to appointments.

LNA II

Education: High school diploma or high school equivalency credential.

Experience: Two years of experience as a Licensed Nursing Assistant.

License/Certification: Must possess and maintain a current license for Nursing Assistant as approved by the New Hampshire Board of Nursing. Must possess a valid driver’s license if responsible to transport residents to appointments. One additional labor grade is provided for Medication Nursing Assistant’s with a valid NH Medication Nursing Assistant License. Employee’s license is reviewed and renewed biennially, in accordance with Board of Nursing Licensing requirements.

SPECIAL REQUIREMENTS:

For promotion from Licensed Nursing Assistant I, pay band 2 to Licensed Nursing Assistant II, pay band 3, employees must:

a. May be required to work non-traditional work hours to meet the needs of the facility;

b. Meet the minimum qualification requirements of the Licensed Nursing Assistant II;

c. Receive satisfactory annual performance evaluations; and

d. Be recommended for promotion by supervisor(s).

Responsibilities:

LNA I

Performs routine duties in meeting the physical and emotional needs of the residents as outlined on the overall Plan of Care.

Documents in resident records any changes, occurrences, and observations on a routine basis.

Guides and assists residents in participating in activities, groups, special functions, appointments, and consultations.

Maintains a clean, safe environment being continuously aware of any risk factors which would effect the residents’ well being. Checks and reports unit supplies, submits needed reports to be done.

Attends in-service educational programs and is responsible to increase own knowledge and keep abreast with current Nursing Assistant duties.

Serves and assists in feeding residents nutritious meals assuring their proper dietary intake and adequate hydration.

Reports all unusual conditions/findings concerning the residents to the supervisor. This includes psychological as well as physiological changes.

Records nursing care activities provided to each resident assigned during his/her shift for accurate documentation of care rendered.

Prepares residents for recreational, educational, and medical trips off grounds and attends trips as required.

May be required to drive residents to medical appointments.

LNA II

Performs and organizes the assignment of groups of residents regarding Activities of Daily Living (ADL) and minor treatment procedures as has been directed by the Charge Nurse.

Provides resident information as needed, assists in basic social problems, and facilitates the adjustment of residents on a daily basis.

Mentors new Nursing Assistants to unit responsibilities according to established policy and procedures to meet the standards of resident care.

Checks and orders ward supplies, submits needed repairs to be done.

Substantiates the need for new clothing for residents; refers other residents’ needs to interdisciplinary team.

Coordinates assistance to residents in performing their ADL including personal hygiene and maintenance of a clean and safe living environment. Documents needed information.

Reports all unusual conditions/findings concerning the residents to the Charge Nurse; this includes psychological as well as physiological changes for further professional evaluation. Participates in Plan of Care meetings as assigned/needed.

Coordinates visits from the consulting professional health team members, including dentists, podiatrists, and physical therapists and provides assistance to them and a level of security and support for the residents.

Attends and completes in-service programs and materials.

May be required to drive residents to medical appointments.

May administer medications, if in possession of a Medication Nursing Assistant License.
