Job Summary:
We are seeking a dedicated and compassionate Clinical Social Worker to provide ambulatory behavioral health services, including evaluation, diagnosis, treatment planning, case management, and psychotherapeutic interventions. The role supports individuals, families, groups, and Intensive Outpatient Programs (IOP). This position offers a flexible work schedule with the opportunity to work hybrid, alongside a generous $6,000 sign-on bonus!
Essential Responsibilities:
- Provide behavioral health services through telephonic triage, crisis management, and telehealth services to members seeking mental health support.
- Conduct thorough psychosocial assessments and develop personalized treatment plans for members.
- Administer telephonic crisis interventions and assessments, determining the appropriate level of care.
- Perform clinical social work consultation, including diagnosis, risk assessments, and crisis intervention.
- Offer psychotherapy and counseling to individuals, families, groups, and IOP participants using evidence-based therapies like cognitive-behavioral therapy (CBT), stress management, family therapy, and more.
- Assess member needs and coordinate referrals to other clinicians, such as psychiatrists or primary care providers.
- Manage post-hospitalization follow-ups, ensuring continuity of care for members after psychiatric admissions or emergency room visits.
- Assist with case management and social work service planning, focusing on personal, social, or economic conditions.
- Collaborate with physicians and interdisciplinary teams to make informed decisions and improve patient outcomes.
- Maintain clinical records, documenting assessments, treatment plans, and interventions in compliance with department and regional guidelines.
Qualifications:
Basic Qualifications:
- Education: Master’s Degree in Social Work from an accredited institution.
- Licensed Clinical Social Worker (LCSW) in Hawaii.
- Basic Life Support (BLS) certification (American Heart Association).
- National Provider Identifier (NPI) required at hire.
- Taxonomy code required at hire.
Preferred Qualifications:
- Experience in primary care behavioral health services.
- Background in addictions counseling.
- Expertise in consultation/liaison services in emergency departments.
- Experience in providing evidence-based treatments for serious mental illness (adults and children).
- Previous work with intensive outpatient programs (IOP) or partial hospitalization programs (PHP).
- Skilled in treatment plan documentation in accordance with national accreditation guidelines.
