Job Description :
We are seeking an experienced Social Work Case Manager with a background in Acute Care Management to join the dynamic team at UMMC. This role involves providing case management services across inpatient units in a Level 1 Trauma Center. Key Responsibilities :
- Provide case management services to acute care patients in a trauma center setting.
- Support a caseload of 15-20 patients per day, addressing medical discharges and social discharges.
- Utilize Epic Systems (EpicCare Inpatient) for charting and documentation.
- Collaborate with multidisciplinary teams, including social workers, nurses, physicians, and discharge planners.
- Conduct assessments to ensure patient readiness for discharge, coordinating complex placement needs.
- Participate in discharge planning and collaborate with the respiratory therapy and pharmacy teams as needed.
- Advocate for patients and families, ensuring smooth transitions through the care continuum.
