Position Summary:
Based on established admission criteria, the Intake and Service Coordinator is responsible for evaluating prospective clients for appropriate treatment and admission into New Hope CORPS programs. This role includes conducting comprehensive screenings and interviews with clients and/or referral sources, identifying needs for detoxification, assessing current emotional and behavioral status, and determining potential risk factors. The coordinator consults with the Program Director to determine final acceptance into the program.
Upon admission, the Intake and Service Coordinator develops a comprehensive service plan that ensures clients have a single point of access to an individualized package of health and social services. This is accomplished through a structured approach involving assessment, planning, linkage to services, monitoring, and client advocacy.

· Conducts thorough screenings and interviews with prospective clients and referral sources to assess clinical and psychosocial needs.
· Evaluates client appropriateness for treatment based on admission criteria, including risk of harm, detox needs, emotional health, and current life situation.
· Collaborates with the Program Director to determine eligibility and readiness for admission into the program.
· Initiates and develops individualized service plans in coordination with the client, outlining tailored health and social service needs.
· Provides care coordination through core case management functions:
o Assessment – Gather in-depth information about the client’s needs and functioning.
o Planning – Develop service strategies and goals in collaboration with the client.
o Linkage – Connect clients with necessary services and supports.
o Monitoring – Track progress, service effectiveness, and modify plans as needed.
o Advocacy – Support clients in accessing and navigating community and healthcare systems.
· Maintains timely and accurate documentation of client assessments, service plans, and coordination activities in compliance with agency policies and regulatory standards.
· Liaise between clients, referral sources, program staff, and external service providers.
qualifications
· Bachelor’s degree in Social Work, Psychology, Counseling, or a related field (Master’s preferred).
· Minimum of 2 years of experience in behavioral health, substance abuse treatment, or related human services field.
· Strong interviewing, assessment, and documentation skills.
· Knowledge of detox protocols, mental health conditions, and risk assessment for harm.
· Demonstrated ability to coordinate and advocate for services in a multidisciplinary environment.
· Excellent interpersonal, organizational, and problem-solving skills.
· Bilingual preferred (English/Spanish or English/Creole).
