What You'll Do at New Hope Health Services as a Certified Addictions Counselor I or II
PLEASE HAVE A VALID DC CERTIFIED ADDICTIONS COUNSLEING! If you are licensed in a different state we are willing to pay for endorsements.
- Be responsible for a caseload of clients, as identified by program requirements and in accordance with DC regulations.
- Complete required evaluations and assessments, including screening, intake, clinical assessments, ASAM and biopsychosocial histories.
- Compete individual treatment plans upon consumer’ admission into the SUD program
- Liaise with SUD Case Worker to ensure that consumer remains in compliance and ensure that basic day to day needs of the consumer are met
- Document all clinical services and interventions performed within the consumers ‘electronic record and complete monthly treatment reports.
- Maintain accurate electronic records on each client and ensure the timely completion of tasks, within 24 hours of service deliver, no later than 72 hours of service delivery.
- Conduct individual and group sessions based on consumers’ treatment plan goals and level of care.
- Participating in funding source reviews and make recommendations for treatment and lengths of stay.
- Monitor consumer progress and adjust treatment plans as needed.
- Coordinate with agency and community resources to ensure a through treatment plan.
- Continually assist clients with developing a discharge/continuing care plan and connecting to resources needed to reach personal goals both during the course of treatment and in advance to their discharge.
- Follow up with clients after discharge based on regulatory guidelines until they have been established with other case management/social services.
- Maintains CACI or II license in good standing, and necessary documentation to HR as required by DC Department of Behavioral Health.
- Desire to grow with New Hope Health Services and suggest/implement new ideas to reach consumers
Position reports to SUD Clinical Director (LICSW), and day to day interaction with CAC Team Lead.
Required: Data Witts. MS Office Knowledge, Group Facilitation, Good Oral and Written Communication, Leadership
Desired: Experience in working with mental health and homeless populations.
Full-Time (40 hours weekly), 90 day probationary period
