Summary of Position :
The Program Case Manager for Shelter / Programs is a full-time exempt position, directly supervised by the Senior Director of Shelter / Programs. S / he supervises Shelter Case Managers. S / he coordinates and provides case management, for guest / residents of the Fairfax operated by New Hope Housing, Inc. Receiving general direction from the Senior Director, s / he is responsible for ensuring the smooth operation and coordination of all case management services in accordance with best practices; working to meet all program goals, targets, and performance outcomes. S / he connects guest / residents with mainstream health and social services, provides training in self-sufficiency skills, assists the guest / residents in meeting their basic needs and advocates with other providers. S / he develops and implements case management programs, including intake processes, case coordination, housing planning, and community case management. S / he oversees and provides supportive supervision to the singles programs case managers; working closely with intern students assigned to the programs.

Some of the essential responsibilities of the Program Case Manager for Shelters / Programs are not limited to :
Utilizing crisis interventions, motivational interviewing, counseling, and problem-solving techniques to engage and establish rapport with guest / residents;Conducting psychosocial assessments to determine immediate need, history, and potential strengths and barriers;Assisting guest / residents in identifying their needs and barriers to maintaining housing stability such as income, credit, physical and mental health issues, substance use, and criminal history.Assisting guest / residents in identifying their strengths and maximizing their personal resources.Developing individualized housing stabilization plans with residents, addressing their service needs in life domains such as housing, family relationships, health care, financial management, education, training, employment, mental health and substance abuse;Referring, coordinating and monitoring the delivery of appropriate services and leveraging community resources to meet residents’ basic and treatment needs;Providing crisis counseling and makes appropriate referrals to assist residents in addressing emotional and environmental problems, including relationship, suicidal ideation, substance use, self-injurious and risky behaviors, immigration and employment.Monitors guest / resident progress and provides supportive services to ensure the housing stabilization, plan goals and strategies meet changing needs and priorities.Accompanying residents to appointments as needed.Coordinating with the Data Quality Coordinator on HUD requirements, reports, and rent payment;Meeting and presenting cases regularly at multidisciplinary team meetings to aid in identifying service needs and engagement techniques.Operating as the primary point of contact on all case-specific issues;Providing guidance and training to interns and volunteers.Coordinating with Operation staff on maintenance repairs.Adhering to the required ethical standards to include confidentiality, boundaries, client rightsCreating and maintaining confidential guest / resident records in compliance with agency policy, to include the use of HMIS for case management, data collection and as a reporting tool. Composes correspondence, maintains records, files, and documents.Provide timely, accurate, and unbiased communication about program-related successes and concerns to the Director of Fairfax Permanent Supportive Housing Programs.When appropriate and agreed upon, providing program leadership in the absence of the Senior Director of Fairfax Permanent Supportive Housing Programs.Provides an early Morning and Late evening work schedule for all Supervised Case Managers.Working flexible hours, including early mornings and late evenings, and weekends every week.Performing related work as required, accepts special projects and other duties as assigned.Preferred Qualifications :
Bachelor’s degree in social work or related field;Extensive knowledge of social, economic, health, and behavioral problems; ability to identify social problems and needs in order to assess residents' ability to utilize available services;Strong knowledge of benefit programs and community resources and ability to interpret laws, policies, and procedures, and regulations for such programs;Ability to establish and maintain effective working relationships and advocate for service population and liaison with community resources;Ability to communicate clearly and effectively both orally and in writing;Completion of college level courses in sociology, psychology, or a related human service field; or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities;Demonstration of commitment to the specific agency mission values and philosophy.Ability to successfully pass a background check and possess a valid driver’s license with a good driving record to be approved by our insurance carrier.Proficient use of computers to include Microsoft Office, Homeless Management Information System (HMIS) and other database software used to track service delivery.Willingness to work early mornings, late evenings and some weekendsThe ability to successfully pass a background check and possess a valid driver’s licenseNature of WorkCoordinates and provides case management, for guest / residents of the Fairfax programs

Minimum RequirementsBachelor’s degree in social work or related field or equivalent years’ experience;
