Promotional opportunity available in our Community Residential Program. The Community Program Coordinator (CPC) provides overall direction and coordination for the Person-Centered Plan, including goals and objectives, advocating for the rights of individuals, and serving as the leader of the New Hope team process. This position prepares reports for all ID team meetings as well as other documents as required. This person will work closely with Community-Based Case Managers.
