Hours: 8am to 5pm, Monday through Friday

Job Description: Under general supervision, the candidate will perform social services functions at an advanced level to individuals and their families. Assist in engagement, needs identification, care coordination, referrals and linkage, and follow-up with individuals on assigned caseload. Skills in documentation, communication with individuals, family, and community resources expected. Must have a valid drivers license and an insured personal vehicle. Travel between Columbus, Ga and Americus, Ga required.
Works with the following population: Individuals with a mental health diagnosis, developmental disabilities, and/or addictive diseases.
Minimum Qualifications: High School Diploma with 3-5 years' experience in behavioral health -or- completion of a bachelor's degree in social work, psychology, mental health, sociology, counseling, or a related discipline. If candidate has a Bachelors, it must be in the above named fields to be MHC eligible.
Preferred Qualifications: A bachelor’s degree and experience working with individuals with mental disorders and/or addictive diseases is preferred.
