Opened in 1986, New Horizons Village is a welcoming apartment complex, offering adults with physical disabilities the opportunity for self-directed living. We are located on 26 picturesque acres in a quiet residential neighborhood in the Farmington Valley. The village is just twenty minutes southwest of Hartford.
New Horizon’s mission is to provide affordable housing, accessible transportation, other support services, short-term and outpatient medical rehabilitation, and long-term skilled nursing in supportive communities of care and enable seniors, adults with disabilities and individuals facing challenges to achieve their goals and live life as independently and joyfully as possible.
PURPOSE OF YOUR POSITION:
Assist the Health Services Coordinator in providing tenants and staff with health and safety education, and performing administrative tasks.
RESPONSIBILITIES INCLUDE:
- Serve as liaison with physicians, homecare agencies, hospital discharge planners, Non-emergency medical transportation and other health service providers to facilitate healthcare management at the tenant's request.
- Triage and report to Health Services RN any health/safety concerns of tenants, staff, and PCAs, providing recommendations for remediation when possible and/or referring to appropriate health professionals at the direction of the RN.
- Assist in the development/revision of policies/procedures regarding health/safety concerns. The Health Services Associate will participate in the NHV Safety Committee as assigned.
- Advocate for quality health services from available resources promoting holistic/preventative healthcare.
- Assist Health Services Coordinator as part of the Tenant Admissions Committee and with all aspects of the admission process.
- As a member of the Health Services Team conduct self professionally and promote teamwork.
- Performs other duties as assigned by your supervisor and/or Village Manager.
WORKING CONDITIONS:
- Works in health services area as well as throughout the facility.
- Sits, stands, bends, lifts, and moves intermittently during working hours.
- Is subject to frequent interruptions.
- Is involved with tenants, personnel, visitors, government agencies, etc., under all conditions and circumstances.
- May be subject to hostile and emotionally upset tenants, family members, personnel and visitors.
- Communicates with the health services staff and other department supervisors.
EDUCATION:
Preferably certified as a Medical Assistant or Nurses Aid. Must be CPR/First Aid certified or obtain within 90 days of hire.
EXPERIENCE:
Experience in a hospital, long-term care facility or other related health care facility working with individuals living with disabilities preferred.
SPECIFIC REQUIREMENTS:
- Must be knowledgeable of medical terminology including diagnoses, procedures, medications, etc.
- Must be highly organized and able to support Health Services Coordinator in variety of administrative tasks including record keeping, file preparation and restocking supplies, first aid kits, etc.
- Must be able to perform CPR and first aid as needed.
- Must be able to relay information concerning a tenant’s condition.
- Must be able to communicate effectively to appropriate personnel regarding emergencies as well as participate in any facility emergency protocols.
- Must have working knowledge of transfer/adaptive equipment and be able to utilize lifts safely and effectively.
- Must be familiar with HIPAA regulations and maintain confidentiality at all times.
- Must be able to push, pull, and do heavy lifting without restrictions up to 51 pounds.
- Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult tenants.
