PURPOSE OF YOUR POSITION
The primary purpose of your position is to assist in the day-to-day activities of the Resident Assessment Process in accordance with current federal, state and local standards, guidelines and regulations that govern our facility under the supervision of the PPS/Case Manager, OBRA, MMQ (MA). The RCCA reports directly to the MDS Director.
EDUCATIONAL REQUIREMENTS
Minimum of a high school diploma, and demonstrated capabilities of clerical skills with an emphasis on organization and ability to prioritize.
EXPERIENCE
Certified Nursing Assistant experience required. Long term care experience preferred. Knowledge of medical terminology, filing, typing, computer and organizational skills.
SCOPE OF RESPONSIBILITY
As the Resident Care Coordinator Assistant you are responsible and accountable to carry out your assigned duties and report directly to the MDS Director. Demonstrates correct safety techniques.
WORKING CONDITIONS
- Works in office areas as well as throughout the facility.
- Sits, stands, bends, lifts, and moves intermittently during working hours. Is subject to frequent interruptions.
- Is involved with residents, personnel, visitors, government agencies, etc., under all conditions and circumstances.
- Is subject to hostile and emotionally upset residents, family members, etc. Communicates with the medical staff, nursing services, and other department advisors.
- Works beyond normal working hours, weekends and holidays, in other positions temporarily when necessary. Scheduled hours may change to meet facility needs.
- Attends and participates in continuing educational programs.
MAJOR DUTIES AND RESPONSIBILITES
Assist in the day-to-day activities of the Resident Assessment Process under the supervision and direction of the PPS/Case Manager, OBRA, and MMQ (MA).
Assist the team in the review and auditing of documentation for accuracy and follow up in- servicing needs.
