Function

The Diabetic Lifestyle Coach and Nutrition Education Coordinator will provide expert guidance and support to individuals at risk of or living with diabetes through comprehensive lifestyle education while implementing nutrition education and obesity programs that promote healthy food choices and physical activity. This position will focus on delivering the National Diabetes Prevention Program (NDPP) and supplement nutrition assistance program education (SNAP-Ed), teaching nutrition, promoting healthy lifestyle habits, and empowering participants to make lasting changes. The role includes facilitating both in-person and web-based workshops, offering personalized coaching, and helping participants connect with healthcare resources to improve their overall health outcomes. The Diabetic Lifestyle Coach and Nutrition Education Coordinator will work independently and in collaboration with other team members to ensure the program’s success. This role requires a commitment to providing culturally competent, evidence-based education with an emphasis on empowerment, lifestyle change, and health improvement.

Responsibilities

  • Diabetes Prevention, Lifestyle Coaching, and Nutrition Education:
  • Deliver the Prevent T2 Program, ensuring all program components are met in alignment with Diabetes Prevention Recognition Program (DPRP) standards.
  • Create individualized action plans for participants, focusing on nutrition, physical activity, weight management, and stress reduction.
  • Provide evidence-based guidance on managing and preventing diabetes, including dietary modifications, healthy habits, and behavior change techniques.
  • Administer evidence-based SNAP-Ed programming to children and adults.
  • Foster a motivating, supportive, and non-competitive environment to inspire lasting lifestyle changes.
  • Establish and maintain strong, trusting relationships with participants, encouraging regular engagement and goal setting.
  • Community Outreach & Education:
  • Organize and facilitate nutrition education workshops in community settings, focusing on topics such as healthy cooking, food budgeting, and exercise.
  • Promote the program and recruit participants through outreach activities, such as community health fairs, workshops, and informational sessions.
  • Program Administration & Data Management:
  • Track participant progress, including attendance, weight, physical activity, and goal achievement.
  • Prepare for each session by reviewing participant progress, lesson plans, and materials.
  • Offer follow-up support to participants who miss sessions, providing makeup options or virtual check-ins. o Maintain accurate records and documentation in compliance with privacy and data security regulations.
  • Collaborative Work:
  • Work closely with the Director of Food Systems and other team members to coordinate and implement program strategies.
  • Support community efforts aimed at improving access to healthy foods and encouraging wellness within local populations.
  • Advocate for public health initiatives and work to reduce health disparities, particularly in communities at risk for diabetes.

Compensation

The salary for this position will be $62,550.76/yr. with a full-time schedule, 35 hours per week. To counter pay inequality and uphold internal parity, we use a nonnegotiable salary system while benchmarking our pay to competitive markets in the nonprofit sector. Excellent benefits include an HMO health plan, long- and short-term disability insurance, life insurance, paid vacation, and a 403(b)-retirement plan. Work hours are from 9am-5pm, with occasional needs for work during weekends and evenings. This position will be on-site at our central office, 2771 Ruth Street, Suite 1, Philadelphia, PA 19134. However, satellite offices will require scheduled work hours at the Engagement Center, 3000 Kensington Avenue, and other NKCDC engagement spaces.

Hiring process

Our hiring process includes a 20–30-minute interview with HR, followed by an in-person interview with the hiring manager, and finally ,an hour-long “panel” interview with 3-6 staff members. The submission of a writing sample(s) may be requested after first in-person interview with the hiring manager. Reference checks are conducted after the final interview.

Requirements

  • High school diploma or equivalent required. A bachelor's degree in nutrition, dietetics, public health, or a related field is preferred.
  • Completion of specialized training to become a CDC Diabetes Prevention Program (DPP) Lifestyle Coach will be required upon hire.
  • 3 to 5 years of experience in health, education, nutrition, exercise science, or another health-related field is preferred.
  • Passion for teaching people about healthy lifestyles and motivating participants to make lasting changes.
  • Solid combination of training, knowledge, and experience in health, nutrition, or exercise program implementation.
  • Skilled in verbal and written communication, well-organized, computer literate, and capable of managing a variety of activities.
  • Ability to work a flexible schedule, including leading classes on nights and weekends.
  • Bilingual/Spanish speaker is preferred.
  • A driver’s license is preferred to access different workshop sites.
  • Knowledge of basic health, nutrition, and fitness principles.
  • Familiarity with public health and community resources and agencies.
  • Ability to work with people from all walks of life and with a variety of stakeholders.
  • Ability to build strong relationships with individuals and foster community within a group setting.
  • Ability to facilitate groups effectively, optimizing social interaction, shared learning, and group cohesion.
  • Ability to understand and address participant safety-related issues in the delivery of the lifestyle program.
