Position Summary :
We are seeking a compassionate and skilled Licensed Social Worker to join our dedicated team. The ideal candidate will be responsible for providing emotional support, advocacy, and resources to our residents and their families. This role requires a deep understanding of social work principles, particularly in the context of long-term care, to promote the well-being and social adjustment of residents.
Key Responsibilities :
- Conduct comprehensive assessments to identify residents’ psychosocial needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Provide counseling and support to residents and families, addressing emotional and social challenges.
- Facilitate support groups and educational workshops for residents and families.
- Advocate for residents’ rights and needs within the facility and the broader healthcare system.
- Assist residents in accessing community resources, benefits, and services.
- Maintain accurate and timely documentation of services provided.
- Participate in care team meetings and contribute to a collaborative care environment.
