As a key member of the interdisciplinary care team, the Director of Social Services will provide emotional support, advocacy, and essential resources to residents and their families while promoting their psychosocial well-being.
Key Responsibilities :
- Comprehensive Assessments : Conduct detailed psychosocial assessments to identify the emotional, social, and mental health needs of residents.
- Individualized Care Plans : Develop, implement, and monitor personalized care plans in close collaboration with the interdisciplinary team, ensuring a holistic approach to each resident's well-being.
- Counseling and Support : Offer one-on-one counseling and group support services to residents and families, addressing challenges such as adjustment, grief, and caregiving stress.
- Advocacy : Advocate for the rights, dignity, and needs of residents within the facility and the larger healthcare system to ensure their well-being is prioritized.
- Community Resources : Assist residents in accessing community resources, services, and benefits, fostering their ability to thrive in the facility and beyond.
- Educational Workshops : Facilitate support groups, educational workshops, and family meetings to equip residents and families with necessary tools and resources.
- Documentation & Compliance : Ensure accurate and timely documentation of all services provided, in line with state and federal regulations.
- Collaboration : Participate actively in care team meetings and contribute to a culture of interdisciplinary collaboration that focuses on holistic care.
