Scaling Community Violence Intervention (CVI) for a Safer Chicago (SC2) is an expanding network of donors, anti-violence practitioners, and community stakeholders dedicated to enhancing CVI in the neighborhoods of Chicago most affected by violence. New Life Centers is partnering with six local community-based organizations; Central States SER, Enlace, Hope Culture, Erie House, Universidad Popular, and Beyond the Ball as The Pathways to Peace. The Pathways to Peace program aim to serve 75% of the highest-risk individuals in the area over five years through comprehensive street outreach, case management, education, employment, and behavioral health services. 

Position Summary 

The Care Navigator position is full-time, 40 hours per week (non-exempt, benefits available) and will take place at our Little Village location. The Care Navigator has a portfolio of participants who have been directly or secondarily affected by street violence to provide case-management, crisis-intervention, support and advocacy. The Care Navigator will work with individuals, families, and friends of victims in coordination with our Street Outreach team. May require some local travel (reimbursable). Pay commensurate with experience. This position is in-person.  

Job Responsibilities 

  • Cooperates with other team members to receive referrals for case management.
  • Provides support and advocacy to the participants through home visits, phone calls, criminal justice advocacy, and referrals to other support organizations.
  • Meets their caseload goals and objectives including linkages to resources.
  • Cooperates with Street Outreach team and NLC partners to provide linkages to resources.
  • Develops relationships with partner organizations and builds “warm referral” network for linkages.
  • Assists data team with data collection and required reporting.
  • Adheres to stipend guidelines and submits required paperwork on time. 
  • Engages effectively with project partners and collaborates to address challenges. 
  • Follows data tracking requirements and meets documentation deadlines. 
  • Submits all finance information by established deadlines. 
  • Attends all training and orientations with partners and funders. 
  • Communicates project information effectively with staff and participants. 
  • Participates in regular meetings with internal team and collaborators. 
  • Responds promptly to requests from the Program Director, Program Coordinator, Data & IT Specialist, and Finance Manager. 
  • Identifies and addresses safety concerns with the NLC team and creates a plan for long-term safety and well-being.
  • Maintains a caseload of 30 participants.
  • Develops service plans through case planning and assessments. 
  • Maintains ongoing discussion with participants to discuss life, school, and work-related goals. 
  • Collaborates with P.O and helps transport and facilitate contact with probation and parole departments as needed. 
  • Attends regular staff meetings to discuss client’s progress. 
  • Develop and support participant engagement and their journey of healing and recovery. 
  • Accurately track attendance and hours. 
  • Provide one-on-one mentorship to participants serving as a positive and responsive role model. 
  • Documents detailed client case-notes and other items as assigned.
  • Maintains accuracy of budget and spending plan.
  • Participates in all required evaluations and audits.
  • Demonstrates New Life Centers’ values.
  • Attends required meetings and training.
  • Other duties as assigned by the Supervisor. 

Skills & Qualifications 

  • Must become a Certified Mandated Reporter. Training provided.
  • Must pass a background check, must not have any pending cases.
  • Must pass a Child Abuse and Neglect Tracking System background check.
  • Must pass a National Sex Offender Registry background check.
  • Must have a working phone and phone number.
  • One year's experience working in the human services field in a capacity that involves direct services to at-risk youth.
  • Cross-cultural competency.
  • Ability to communicate effectively both written and verbally.
  • Proficient in Microsoft Word and Excel.
  • Must be willing to be trained, and proficiently utilize database(s) and information tracking systems relevant to duties.
  • Must keep confidentiality of participant information.
  • Ability to establish and maintain effective working relationships with others.
  • Valid Illinois driver’s license, insurance, good driving record, access to a vehicle. Subject to driving background check.
  • Bilingual English/Spanish preferred but not required.
