JOB TITLE: Reentry Coordinator

WAGE: Competitive, based on education and experience $48k-65k per

WORK HOURS: Variable eight hours per day scheduled at discretion of Programs Manager
GENERAL SUMMARY:
· The Reentry Coordinator facilitates the successful transition of New Mexico’s incarcerated population back into the community by working closely with correctional facility staff, outside community resources and other vital stakeholders to create an effective bridge pre to post-release. Specialized training in the COMPAS tool allows the Reentry Coordinator to provide an objective risk and needs assessment and standard for evidence-based case management and referrals. Based off an established case plan, Reentry Coordinator’s organize workshops and facilitate in-reach efforts which address individual’s criminogenic risks and needs and enhance the probability of a successful transition out into the community. While focusing on prerelease efforts, the continuum of care is vital to this position and works to coordinate services with Probation and Parole and other needed supports within the community prior to release. MINIMUM QUALIFICATIONS:
· High School diploma, GED certification or equivalent is required
PREFERRED EXPERIENCE:
· 6 months experience related to purpose of position
· Prior experience working in Reentry services working with criminal justice population
· Prior teaching or training experience
· Case Management experience
WORK ENVIRONMENT:
· 90% indoor – 10% outdoor. Work is done inside the perimeter of an adult prison environment and in a variety of settings in a correctional institution, and includes daily contact with and supervision of inmates, sometimes one-on-one, and is potentially a very dangerous environment. Work is performed in an office and classroom setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer usage. Some travel may be required.

70% rapid pace to meet deadlines – 30% at own pace
ESSENTIAL DUTIES:
· Conducts Risk and Needs assessments
· Develops Case Plans for individuals transitioning out of incarceration based off the RnR assessment
· Coordinates with the Community Outreach and Faith-based Services Coordinator and PPD Transitional Coordinators
· Facilitates in-reach efforts with outside community organizations and other vital stakeholders
· Secures Vital documents for individuals prerelease
· Provides Identification and Driver’s Licenses utilizing the MVD Workstations
· Builds and provides Reentry Packets to individuals prerelease
· Works with Managed Care Organizations to assist in Medicaid Applications
· Works with the Social Security Administration and other agencies for vital records
· Refers individuals to Peer Support as needed
· Assist inmates with work readiness
· Perform related duties as required and assigned

EMPLOYMENT REQUIREMENTS:
· This position is designated as safety sensitive.

· Must possess and maintain a valid New Mexico Driver's License.
· Must be legally authorized to work in the U.S and have reached the age of majority.
· Willingness to work under potentially dangerous circumstances in a correctional facility and maintain strict fidelity to security protocols.
