Why does the job exist?

This position will provide effective advocacy for long-term care residents and acts as a liaison with internal and external entities, ensuring resident rights are not violated and resident and family concerns are addressed. Will provide comprehensive resident assessments, as well as care and discharge planning.

How does it get done?

Advocates & assists resident to attain, or maintain his/her highest level practicable well-bing

Perform medically related social services needs in accordance with the State, Fedral and Veteran Regulations,

Complete comprehensive resident assessments and care plans

Document in computer resident needs, requests, appointments and other necessary information.

Reviews grievances and resident/family concerns and takes appropriate steps towards resolution.

Assess for Trauma and develop care plans to meet residents needs and monitor for PTSD symptoms

Participate in pre-admission assessment process

Facilitate discharge planning process with resident and Interdisciplinary team members.

Participate in Psychopharmacology Review team to assess resident emotional, mental and cognitive impairments and related medicaiton.

Who are the customers?

Residents of a Long-Term Care Facility . Resident who are admitted for short term skilled care with anticipation to discharge back to community. Work with families and/or legal representatives, insurance care coordinators,community medical providers, community therapy providers, medical equipment providers, and veteran's services.

Ideal Candidate

Will have: Bachelor's or Masters Degree in Social work. Has a current & active Social Work License from the State of NM.

The ability to read, write, speak, and understand the English language.

The ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

The ability to function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.

Must maintain current NM Social Work License.

Minimum Qualification

Associate's degree in Human Services or Social Sciences and two (2) years experience in mental health and/or substance abuse counseling. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience.

Employment Requirements

This position is designated as safety sensitive. Employment is subject to pre-employment and random testing in accordance with all terms and conditions of federal and state law, rules and regulations in relation to alcohol and/or drug testing. Employment is subject to nationwide criminal background check and is conditional pending results. Successful completion of the Health Insurance Portability and Accountability (HIPPA) training.

Working Conditions

Position will perform work primarily in an office setting with extensive computer and phone usage and exposure to Visual/Video Display Terminal (VDT). Exposure to potentially violent hard-to-manage patients in volatile situations. Some travel may be required. May be required to work weekends, holidays and overtime.

State of New Mexico employees may be impacted by the Public Health Order dated December 19, 2022, which requires facilities licensed or certified by the Centers for Medicare and Medicaid Services ("CMS"), including all hospital types, long-term care facilities, nursing homes, hospice facilities and rehabilitation facilities, as well as assisted living facilities and adult day care settings, to adhere to all COVID-related requirements prescribed by CMS, including, but not limited to masking and staff vaccination.

Works in office areas, resident rooms, etc.

Moves intermittently during working hours.

Is subject to frequent interruptions.

Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.

Communicates with the medical staff, nursing personnel, and other department directors.

Works beyond normal working hours and on weekends and holidays when necessary.

Must be constantly alert for resident safety.

Is subject to exposure to infectious waste, diseases, conditions, etc.

Is subject to call back during emergency conditions.

Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet.

Bargaining Unit Position
