Why does the job exist?
Incumbent will be responsible for primary functions of this position are a Social Worker overseeing discharge planning activities for all patients served within one of the Meadows unit one of our Long Term Care (LTC) facilities.
How does it get done?
Incumbent will compile and complete a Social Services assessment on admission, and as needed by collecting data from resident, resident's family, and/or other sources.

Ideal Candidate
Ideal candidate will have:
Experience working with treatment planning;
Experience working with providing direct effective treatment to patients;
Experience working with various psychotherapeutic modalities;
Experience working with mental ill;
Must be licensed in accordance with the Social Work Practice Act, Sections 61-31-1 through 61-31-25 NMSA 1978, and 16.63.1 through 16- 63-20 NMAC.
Minimum Qualification
Associate's degree in Human Services or Social Sciences and two (2) years of experience in mental health and/or substance abuse counseling. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience.

Effective 01/01/2015, the New Mexico Behavioral Health Institute became a 100% Tobacco Free/E-Cigarette Free Campus. As such, no tobacco, tobacco products or E-Cigarette products are permitted on any part of the campus. This position is within the Long Term Care/ Meadows. Work hours are 8 am to 5 pm.
